小切口非超声乳化摘除术治疗短眼轴性白内障

摘要 目的分析短眼轴性白内障小切口非超声乳化摘除术的特点 ,探讨其并发症的预防措施。方法对行小切口非超声乳化的白内障摘除术、眼轴长度为 19 5 6 - 2 3 OOmm的白内障患者 118例 (14 3只眼 )进行回顾性分析。结果术后 1个月矫正视力≥ 1 0者 2 8只眼 (19 6 % ) ,0 5 - 0 9者 10 2只眼 (71 3% ) ,0 3- 0 4者 13只眼 (9 1% ) ;术中晶状体后囊膜破裂 8只眼 (5 6 % ) ,术后角膜可逆性水肿 38只眼 (2 6 6 % )。随访过程中眼压升高者 2只眼 (1 4 % )。结论短眼轴性白内障患者前房浅、眼前段组织空间狭小、玻璃体压力高 ,手术难度大 ;强调术前降低眼内压 ,术中规范切口 ,对硬核应用咬核或劈核技术 ,合理使用高质量粘弹剂 ,保持前房深度 。 Objective To analyze the operative characteristic of cataract extraction non-phacoemultification through a small-incision in a short eye axis and discuss its preventive measures of the complications.Methods A retrospective study was performed to analyze the 118 patients (143eyes) who had undergone cataract extraction by non-phacoemulsification throug a small-incision and whose ocular axial lenths were 19.56-23.0mm.Results Postoperatively, the visual acuities were ≥1.0 in 28 eyes (19.6%),0.5-0.9in 102 eyes (71.3%)and 0.3-0 4 in 13 eyes (9.1%)after 1 month. The posterior capsular rupture was 8 eyes (5.6%) in the operation; The postoperative reversible corneal edema was 38 eyes (26.6%).During the postoperative follow-up, the intraocular pressure going up was 2 eyes (1.4%).Conclusion Because the patients had shorter ocular axis, shallower anterior chamber depth ,narrower space in the anterior ocular segment and higher vitreous pressure,the operation was hard to perform.It is the key procedures to obtain satisfactory operative results to emphasize the reduction of the intraocular pressure before operation, to use the correct technique of incision in the operation, to apply the technique of biting and chopping nucleus to hard nucleus with the help of viscoelastic material and to maintain deep anterior chamber.
