摘要 目的探讨冠心病(CHD)患者外周血中肿瘤坏死因子样弱凋亡因子(TWEAK)及其受体可溶性CD163的表达水平及其临床意义。方法选取2015年1月~2016年1月于延安大学附属医院东关心血管分院收治的154例经冠状动脉造影确诊的CHD患者(病例组)、男性98例,女性56例,平均年龄64.8±10.2岁;另选取50例为正常研究对象(正常组),男性33例,女性17例,平均年龄62.3±11.0岁;采用酶联免疫吸附试验(ELISA)检测外周血中可溶性TWEAK、可溶性CD163的水平。结果急性心肌梗死(AMI)、不稳定性心绞痛(UAP)、稳定性心绞痛(SAP)患者和正常组外周血中可溶性TWEAK、可溶性CD163水平差异具有统计学意义(P<0.05);AMI患者的外周血中可溶性TWEAK、可溶性CD163显著的高于UAP、SAP患者和正常组(P<0.05),UAP、SAP患者的外周血中可溶性TWEAK、可溶性CD163显著的高于正常组(P<0.05);AMI、UAP、SAP患者的Gensini评分呈逐渐降低趋势且差异具有统计学意义(P<0.05)。CHD患者外周血中可溶性TWEAK、可溶性CD163水平与Gensini评分均呈显著的正相关关系(P<0.05)。结论CHD患者外周血中可溶性TWEAK、可溶性CD163水平显著的升高,并且与患者病情严重程度相关。 Objective To investigate the expression and clinical significance of TNF-like weak inducer of apoptosis (TWEAK) and its receptor soluble CD163 in peripheral blood of patients with coronary heart disease (CHD).Methods 154 CHD patients diagnosed by coronary angiography between January 2015 and January 2016 from Dongguan Branch of Yan'an University Affiliated Hospital with in patients were selected as case group, including 98 males, 56 females, with mean age of 64.8±10.2 years. 50 normal subjects were selected as control group, including 33 males and 17 females, with mean age of 62.3±11.0 years. The levels of soluble TWEAK and soluble CD163 in peripheral blood was detected by ELISA method. Results Levels of soluble TWEAK and soluble CD163 in peripheral blood of acute myocardial infarction (AMI) patients, unstable angina pectoris (UAP) patients, or stable angina pectoris (SAP) patients, patients of normal group were significantly different (P<0.05). Levels of soluble TWEAK and soluble CD163 of AMI patients were significantly higher than UAP and SAP patients and normal group (P<0.05). Levels of soluble TWEAK and soluble CD163 of UAP and SAP patients were significantly higher than normal group (P<0.05).The Gensini score of AMI, UAP and SAP patients showed a gradual decreasing trend and were significantly different (P<0.05). There was a significant positive correlation between soluble TWEAK, soluble CD163 level and Gensini score in peripheral blood of CHD patients (P<0.05). Conclusion Peripheral blood soluble TWEAK, soluble CD163 were significantly elevated in CHD patients, and were related to the severity of the patient's condition.
