摘要
目的观察采用玻璃体腔注射曲安奈德治疗全视网膜光凝后继发黄斑水肿的临床效果。方法 84例糖尿病视网膜病变经全视网膜光凝后继发黄斑水肿患者,按照随机数字表法分为观察组和对照组,各42例。观察组患者采用玻璃体腔注射曲安奈德治疗,对照组患者口服羟苯磺酸钙胶囊治疗。比较两组患者的治疗效果。结果观察组治疗后各时间点最佳矫正视力均高于治疗前,差异具有统计学意义(P<0.05)。对照组治疗3、6个月最佳矫正视力高于治疗前,差异具有统计学意义(P<0.05)。观察组治疗后各时间点最佳矫正视力均高于对照组,差异有统计学意义(P<0.05)。观察组眼压治疗前(15.34±3.55)mm Hg(1 mm Hg=0.133 k Pa)、治疗后1周(17.63±3.19)mm Hg、治疗后1个月(16.32±3.02)mm Hg、治疗后3个月(15.21±2.95)mm Hg、治疗后6个月(14.58±2.75)mm Hg;对照组眼压治疗前(15.56±3.81)mm Hg、治疗后1周(15.30±3.51)mm Hg、治疗后1个月(15.27±3.32)mm Hg、治疗后3个月(14.82±4.06)mm Hg、治疗后6个月(14.52±3.63)mm Hg。观察组治疗后1周眼压高于其他时间段眼压,差异具有统计学意义(P<0.05);对照组眼压持续下降,但差异无统计学意义(P>0.05)。观察组治疗后各个时间点黄斑中心凹厚度均低于对照组,差异有统计学意义(P<0.05)。结论玻璃体腔注射曲安奈德治疗全视网膜光凝后继发黄斑水肿后最佳矫正视力与黄斑中心凹厚度均改善显著,但术后可出现短期眼压升高,临床应引起重视。
Objective To observe clinical effect by triamcinolone acetonide by intravitreal in jection inthe treatment of secondary macular oedema after panretinal photocoagulation.Methods A total of84patients with secondary macular oedema after panretinal photocoagulation for diabetic retinopathy were divided by random number table into observation group and control group,with42cases in each group.The observation group received triamcinolone acetonide by intravitreal injection for treatment,and the control group received oral administration of calcium dobesilate capsules for treatment.Comparison was made on curative effects between the two groups.Results The observation group had all higher best corrected visual acuity in each time point after treatment than the control group,and their difference had statistical significance(P<0.05).The control group had higher best corrected visual acuity in3and6months after treatment than that before treatment,and the difference had statistical significance(P<0.05).The observation group had all higher best corrected visual acuity in each time point aftertreatment than the control group,and their difference had statistical significance(P<0.05).The observation grouphad intraocular pressure as(15.34±3.55)mm Hg(1mm Hg=0.133kPa)before treatment,(17.63±3.19)mm Hgin1week after treatment,(16.32±3.02)mm Hg in1month after treatment,(15.21±2.95)mm Hg in3monthsafter treatment and(14.58±2.75)mm Hg in6months after treatment.The control group had intraocular pressureas(15.56±3.81)mm Hg before treatment,(15.30±3.51)mm Hg in1week after treatment,(15.27±3.32)mm Hgin1month after treatment,(14.82±4.06)mm Hg in3months after treatment and(14.52±3.63)mm Hg in6months after treatment.The observation group had higher intraocular pressure in1week after treatment than inthe other time point,and the difference had statistical significance(P<0.05).The control group showed graduallydecreasing intraocular pressure,and the difference had no statistical significance(P>0.05).The observation grouphad all lower central fovea of macula thickness in each time point after treatment than the control group,and theirdifference had statistical significance(P<0.05).Conclusion Triamcinolone acetonide by intravitreal injection inthe treatment of secondary macular oedema after panretinal photocoagulation provides remarkable improvementsin best corrected visual acuity and central fovea of macula thickness,however short-term intraocular pressureincreasing may occur after operation,and it requires clinical attention.
