摘要
许多研究已证实椎间盘有神经分布,然而,多数学者都集中在腰椎间盘神经分布的研究上。颈椎间盘的神经分布与腰椎间盘类似,后方来自窦椎神经,侧方来自椎体神经,前方来自交感干。支配颈椎间盘的神经可分为感觉神经和自主神经。椎间盘中包含的机械感受器主要是游离神经末梢和鲁菲尼小体,故椎间盘与伤害性刺激有关,颈椎间盘可能是颈痛的原因。颈椎间盘相关性疼痛受窦椎神经调节,交感神经也参与其中。与颈椎病相关的临床症状包括眩晕、头痛、耳鸣、恶心呕吐、视力模糊、心悸等。总之,颈椎间盘神经丰富,鲁菲尼小体长入病变的颈椎间盘会导致前庭功能紊乱,产生眩晕;高血压也与颈椎病相关,通过颈椎减压手术可有效治疗伴发的眩晕及高血压。
Numerous studies have found that the intervertebral disc(IVD)is innervated.However,most of investigators havefocused on the lumbar discs.Innervation in cervical disc is analogous to that in the lumbar spine,receiving innervation posteriorly fromthe sinuvetebral nerves,laterally from the vertebral nerve,and anteriorly from the sympathetic trunks.Neurons innervating cervical IVDscan be divided into sensory neurons and autonomic neurons.Mechanoreceptors that containing human IVDs are mainly free nerve endingsand Ruffini corpuscles,so IVDs are associated with nociception.IVDs may be the source of neck pain.Pain related to IVDs is mediatedthrough the sinuvertebral nerve,the sympathetic nerve is also involved.Some symptoms related to cervical spondylosis include vertigo,headache,tinnitus,nausea and vomiting,blurred vision,and palpitations.In conclusion,cervical IVDs are fully innervated;growth of Ruffinicorpuscles in the diseased cervical disc can lead to vestibular dysfunction and produce vertigo.Hypertension is also associated with cervicalspondylosis,cervical decompression surgery can cure concomitant vertigo and hypertension effectively.
