摘要
目的:探讨Y-V推进式改良新月形皮瓣去除内眦赘皮并同期行重睑术在临床中的应用效果。方法:本组筛选42例内眦赘皮合并单睑患者为研究对象,采用Y-V推进改良新月形皮瓣去除内眦部新月形皮肤,打断错构附着于内眦韧带的眼轮匝肌,对内眦韧带行缩短固定,同时行切开法重睑术;并采集术前、术后照片,测量相关美学参数,对资料行对比分析。结果:术后随访3~15个月,15例术后1周拆线时切口已无红肿,18例在术后1周内眦切口发红,7例术后第10d内眦处切口轻度瘢痕增生,2例术后1月出现明显瘢痕,给予相关处理后半年随访时均未见明显瘢痕;与术前对比所有患者内眦间距平均缩短7.84%,睑裂横径平均增加4.94%,差异有统计学意义(P<0.05)。结论:Y-V推进改良式新月形皮瓣去除法内眦赘皮矫正术是矫正内眦赘皮的有效方法,同期结合重睑形成术眼部整体形态更佳,值得临床应用推广。
Objective To explore the clinical effect of epicanthoplasty by modified crescent flap removal combined with Y-V for correction of epicanthus and double eyelid plasty.Methods In this group,42cases of epicarditis combined with single eyelid patients were selected as the research object.The preoperative and postoperative photographs were collected and the related aesthetic parameters were measured,and the data were analyzed before and after the data.Modified the crescent flap simultaneous with Y-V removed was used to remove the inner canthus crescent-shaped skin,interrupted by the wrong structure attached to the canthus ligament of the orbicularis oculi muscle,the inner canthal ligament line to shorten the fixed,double eyelid surgery was done concurrently.Results The patients were followed up for3months to15months.In15cases,the incision was no longer inflamed in one week after operation,18cases had redness in incision at one week after operation,7cases the incision have mild scar hyperplasia the ten days after and2cases had obvious scar at1month postoperatively.No obvious scar was found in the6months follow-up.All the patients had an average7.84%reduction in inner canthaldistance(P<0.05)and an average increase of4.94%in blepharoplasty(P<0.05),the difference was statistically significant.Conclusion The improved method of modified crescent flap removal by Y-V is an effective method for correction of epicanthus.It is worthy to be popularized in the clinical application of eyelid forming.
