摘要
目的:探讨补肾疏肝法治疗肝郁肾虚型成年女性痤疮的临床疗效及对基础性激素6项—血清睾酮(T)、雌二醇(E2)、泌乳素(PRL)、黄体生成素(LH)、和卵泡刺激素(FSH)、孕酮(PGN)的影响。方法:将符合纳入标准的痤疮患者共70例按随机数字表法随机分为治疗组及对照组,每组35例,治疗组采用自拟疏肝补肾汤治疗,对照组以螺内酯口服,两组均同时给予甲硝唑软膏外涂。两组治疗4周后,观察两组治疗前后临床疗效及性激素水平的变化。同时测定相同年龄组健康女性30例的基础性激素作为对照。结果:入选的病例组与同龄健康组比较,基础性激素对比无显著性差异;治疗4周后,中药组的皮损疗效与对照组相比有效率存在统计学意义,中药组有效率优于对照组。且中药组在改善中医症状方面优于对照组。治疗组与对照组性激素水平相互之间比较及两组在治疗前后比较均无显著性差异(P>0.05)。结论:肝郁肾虚证成年女性痤疮不存在血清性激素的变化,疗效机理与性激素平衡的调节无关,与分析可能通过免疫调节达到治疗效果。
Objective:To observe the clinical efficacy of nourishing kidney and dredging liver therapy in treating adult female acne patients with stagnation of liver Qi and deficiency of kidney-Yin and its effect on testosterone(T),estradiol(E2),prolactin(PRL),luteinizing hormone(LH),follicle stimulating hormone(FSH)and progesterone(PGN).Methods:70adult female acne patients who met the inclusion criteria were randomly divided into the treatment group and the control group according to the random number table,with35cases in each group.The treatment group received the self-made traditional Chinese medicine decoction of nourishing kidney and dredging liver;the control group received spironolactone,and both groups received external use of Metronidazole Cream.Clinical efficacy and the levels of sex hormone of two groups were observed4weeks after treatment,and the levels of30similarly aged healthy women’sex hormone were taken as a control.Results:There was no significant difference in basic sex hormone between the healthy group and patients’group.The effective rate of the treatment group was higher than that of the control group,the difference was statistically significant.And the treatment group’s curative effect in improving TCM symptoms was significantly better than that of the control group.There were no significant differences in sex hormone between the treatment group and the control group,and their own comparisons before and after treatment.Conclusions:The levels of the basic sex hormone of adult female acne with stagnation of liver Qi and deficiency of kidney-Yin were not abnormal.The mechanism of the self-made traditional Chinese medicine decoction of nourishing kidney and dredging liver was not related to regulating sex hormone levels,but may be related to immune regulation.
