摘要
目的分析血浆超敏C反应蛋白(hs-CRP)、B型脑钠肽(BNP)检测老年慢性心力衰竭(CHF)患者院内恶性室性心律失常发生的临床应用意义。方法 40例CHF恶性室性心律失常老年患者作为观察组,40例CHF非恶性室性心律失常老年患者作为对照组,测定并对比两组24 h内的hs-CRP与BNP水平。结果观察组的左室射血分数(LVEF)为(38.28±4.37)%,低于对照组的(45.62±5.53)%,差异有统计学意义(P<0.05)。观察组的hs-CRP为(9.22±3.14)mg/L、BNP为(1895.17±310.74)pg/ml,均高于对照组的(3.74±1.15)mg/L、(1352.18±257.83)pg/ml,差异有统计学意义(P<0.05)。经多因素回归分析证实,CHF老年患者院内恶性室性心律失常发生的主要因素为hs-CRP、BNP和LVEF。高浓度hsCRP和BNP发生恶性室性心律失常的患者比例显著高于低水平hs-CRP和BNP患者,差异有统计学意义(P<0.05)。结论 hs-CRP、BNP是判断老年CHF患者发生院内恶性室性心律失常的主要因素,且水平越高,发生率越高。
Objective To analyze the clinical significance of plasma high sensitivity C reactive protein(hs-CRP)and type B brain natriuretic peptide(BNP)in the diagnosis of malignant ventricular arrhythmias inhospitalized elderly patients with chronic heart failure(CHF).Methods There were40CHF patients withmalignant ventricular arrhythmias as observation group,and40CHF patients with non-malignant ventriculararrhythmias as control group.Detection and comparison were made on hs-CRP and BNP level in24h in twogroups.Results The observation group had lower left ventricular ejection fraction(LVEF)as(38.28±4.37)%than(45.62±5.53)%in the control group,and the difference had statistical significance(P<0.05).The observationgroup had higher hs-CRP as(9.22±3.14)mg/L and BNP as(1895.17±310.74)pg/ml than(3.74±1.15)mg/L and(1352.18±257.83)pg/ml in the control group,and their difference had statistical significance(P<0.05).Multivariate regression analysis confirmed that the main factors of CHF elderly patients with malignant ventriculararrhythmias were hs-CRP,BNP and LVEF.The proportion of patients with high concentrations of hs-CRP andBNP with malignant ventricular arrhythmias was significantly higher than that of low-grade hs-CRP and BNPpatients,and the difference had statistical significance(P<0.05).Conclusion hs-CRP and BNP are the mainfactors in judging the incidence of malignant ventricular arrhythmia in elderly CHF patients,and the higher thelevel,the higher the incidence.
