摘要
目的:探讨维持性血液透析中心静脉留置导管患者应用双腔负压吸引能否将透析导管静脉腔内的不牢固血栓清除至体。方法:采用自身前后对照研究方法,将8例维持性血液透析的长期留置导管患者前后各两个月两种上机方法进行对照评价,前两月为对照组(8例,180例次),后两月为实验组(8例,158例次,因中途有1例患者死亡,所以少22例次)。对照组采用单腔负压吸引法上机,利用血泵从透析导管的动脉管腔引出血液;实验组采用双腔负压吸引法上机,于上机前先利用透析机血泵从透析导管的静脉管腔引出血液,然后再将血液回路管的动脉端换至透析导管动脉端进行透析,实现对透析导管的动、静脉双腔均进行负压吸引。结果:实验组血栓清除率为4.43%,比对照组(2.22%)高2.21倍;两组透析器凝血情况比较,差异无统计学意义(P>0.05)。结论:双腔负压吸引法对透析导管内不牢固血栓的清除效果优于单腔负压吸引法,同时并不增加透析器的凝血。
Objective:To investigate the application of double cavity pressure suction in eliminating thrombus in the central venous duct in hemodialysis patient.Methods:Auto case-control study was used in this experiment;8cases of patients with long-term indwelling catheter maintenance hemodialysis were selected.And in thefirst two months after experiment began(8cases,180case-times)it was observed as control group;in the second two months after experiment began(8cases,158case-times,for one case was dead in the midway,so22case-times was missed)it was observed as observation group;the looming method was comparedbetween two groups.The conventional operation method was used in control group;the single cavity pressure suction method was used in hemodialysismachine utilization;the blood pump was used in vacuum aspiration of cavumateriae of dialysis catheter.The double cavity pressure suction method was usedin hemodialysis machine utilization in observation group,on the basis of in the control group,the blood pump was used in veinaspiration of cavumateriae ofdialysis catheter too.Results:The clearance rate that in control group(4.43%vs.2.22%);the condition of dialyser coagulation had no significantly differencebetween two groups(P>0.05).Conclusion:Comparing with single cavity pressure suction method,double cavity pressure suction method has better effect ineliminating thrombus and has no risk in increasing dialyser coagulation.
