摘要
目的:探究远红外线烤灯照射与尿激酶静脉溶栓联合腔静脉滤器置入术对烧伤并发下肢深静脉血栓形成患者生活质量的影响。方法:选取2013年5月~2016年12月郑州市第一人民医院44例烧伤并发下肢深静脉血栓形成患者,依据治疗方案不同分组,各22例。对照组施行远红外线烤灯照射+腔静脉滤器置入术治疗,观察组施行远红外线烤灯照射+腔静脉滤器置入术+尿激酶静脉溶栓治疗。统计两组治疗效果,并对比治疗前后两组健、患侧下肢周径差及生活质量变化情况。结果观察组治疗总有效率100.00%(22/22)远高于对照组72.73%(16/22),差异有统计学意义(P<0.05);两组治疗前健、患侧大腿、小腿周径差相比,差异均无统计学意义(P>0.05);相较于对照组,观察组治疗后健、患侧大腿、小腿周径差均较小,差异有统计学意义(P<0.05);两组治疗前各维度生活质量评分相比,差异均无统计学意义(P>0.05);相较于对照组,观察组治疗后各维度生活质量评分均较高,差异有统计学意义(P<0.05)。结论:对烧伤并发下肢深静脉血栓形成患者予以远红外线烤灯照射与尿激酶静脉溶栓联合腔静脉滤器置入术治疗,可显著缓解患者下肢肿胀,提高生活质量,效果较为显著。
Objective:To investigate the effect of far infrared ray irradiation and urokinase intravenous thrombolysis combined with vena cava filteron quality of life in patients with deep venous thrombosis of lower extremities.Methods:From May2013to December2016,44patients with burnedlower extremity deep venous thrombosis in Zhengzhou First People's Hospital were divided into control group and observation group accordingto treatment plan,22cases each group.The control group was treated with far infrared ray lamp irradiation and vena cava filter implantation.Theobservation group was treated with far infrared lamp irradiation+vena cava filter implantation and urokinase intravenous thrombolytic therapy.Treatment effect of the two groups was analyzed;healthy,ipsilateral lower limb circumference difference and quality of life of the two groups beforeand after treatment were compared.Results:The total effective rate was100.00%(22/22)in the observation group,which was significantly higher thanthat in the control group(72.73%,16/22),the difference was statistically significant(P<0.05);Before treatment,there was no significant differencein healthy,ipsilateral lower limb circumference difference between the two groups(P>0.05);Compared with the control group,the ipsilateral thigh,calf circumference difference of the observation group after treatment were small,the difference was statistically significant(P<0.05);There was nosignificant difference in the quality of life scores between the two groups before treatment(P>0.05);Compared with the control group,the quality oflife scores of the observation group were higher after treatment,the difference was statistically significant(P<0.05).Conclusion:Treatment of burnedwith lower extremity deep vein thrombosis patients using far infrared ray lamp irradiation and urokinase intravenous thrombolysis combined withvena cava filter placement surgery can significantly alleviate the lower extremity swelling,improve the quality of life,the effect is more significant.
