摘要
目的研究分析恩替卡韦联合阿德福韦酯治疗慢性乙肝的效果及其作用机制。方法选取本院在2014年1月~2016年1月间收治的128例拉米夫定耐药性慢性乙肝患者为研究对象,随机等分为对照组和实验组,对照组采取拉米夫定联合阿德福韦酯的治疗方案,实验组采取恩替卡韦联合阿德福韦酯的治疗方案,两组均连续给药48周,比较两者患者的各项指标。结果治疗后12、24、48周较治疗前,两组患者的血清HBV DNA水平均明显下降(P<0.05),且实验组各时间节点的血清HBV-DNA水平明显低于对照组(P<0.05);与治疗前比较,治疗后各时间节点患者血清ALT及AST水平比较差异具有统计学意义(P<0.05),且实验组各时间节点患者血清ALT及AST水平明显低于对照组(P<0.05);实验组治疗后各时间节点HBV-DNA转阴率、HBe Ag转阴率、病毒学应答率均明显高于对照组(P<0.05)。以上各数据比较差异具有统计学意义。结论恩替卡韦联合阿德福韦酯治疗拉米夫定耐药性慢性乙肝具有较理想的治疗效果,值得在临床上推广应用。
Objective To study the effect of entecavir combined with adefovir dipivoxil on chronic hepatitis B and its mechanism.Methods128patients with chronic hepatitis B were treated with lamivudine-resistant chronic hepatitis B from January2014to January2016.The patients were randomly divided into control group and experimental group.The control group was treated with lamivudine Adefovir dipivoxil treatment regimen,the experimental group to take entecavir combined with adefovir dipivoxil treatment program,the two groups were continuous administrationof48weeks,compared the two patients with the indicators.Results The levels of serum HBV-DNA in the two groups were significantly decreased(P<0.05)at12,24and48weeks after treatment,and the levels of serum HBV-DNA in the experimental group were significantly lower than those in the control group(P<0.05).Compared with the control group,the levels of serum ALT and AST in the patients after treatment were significantly lower than those in the control group(P<0.05).The HBV-DNA negative rate,HBeAg negative rate and virological response rate were significantly higher in the experimental group than in the control group(P<0.05).The above data were significantly different,with statistical significance.Conclusion Entecavir combined with adefovir dipivoxil in the treatment of lamivudine-resistant chronic hepatitis B has an ideal therapeutic effect and is worthy of clinical application.
