摘要
目的建立适合贵阳市市级医院院前创伤急救的规范流程,提升严重创伤/交通伤的救治效率和质量。方法组织专家团队,根据贵阳市创伤院前急救需求、现况与资源等,制定市级医院"创伤院前急救流程和操作规范",并通过反复模拟推演,完善流程和技术规范。采集和比较规范实施前后的严重创伤急救效率和结局指标。结果本规范实施后,我院严重创伤急救的院前急救派出时间较实施前缩短41.36%[(2.24±0.84)min VS(3.82±1.02)min,P<0.001],急诊通过时间缩短50.16%[(20.57±7.96)min VS(41.27±11.02)min,P<0.001],第一次确定专科治疗时间缩短37.59%[(49.59±18.19)min VS(79.46±25.57)min,P<0.001],院内死亡人数从实施前的2人降为实施后的0人(P<0.001);院前急救到达时间和送达时间从无到有,院前与院内信息联动达100%。结论本研究建立的市级医院院前急救流程规范,为医院院内救治赢得时间,对降低严重创伤的死亡率和致残率将起到重要的作用。
Objective To establish the standard and work flow of pre-hospital trauma rescue for metropolitan hospitals inGuiyang city so as to improve the efficiency and quality of severe/traffic trauma rescue.Methods An expert team wasrecruited to create the work flow and operation standards of pre-hospital rescue for metropolitan hospitals in Guiyang city onthe basis of the current status,needs,and resources.Then the standards were practiced repeatedly and revised.The efficiencyand outcome data of severe trauma rescue were collected and analyzed before and after the application of the standards.Results After application of the standards,the rescue dispatch time was shortened by41.36%[(2.24±0.84)min VS(3.82±1.02)min,P<0.001];the time spent in emergency department was shortened by50.16%[(20.57±7.96)min VS(41.27±11.02)min,P<0.001];the time of first referral to specialist was shortened by37.59%[(49.59±18.19)min VS(79.46±25.57)min,P<0.001];There was no death after work flow was applied whereas there were two death happened before the application ofthe work flow.The duration of delivery and arriving time were recorded in the work flow.The synchronization of pre-hospital and hospital data was performed at100%level.Conclusion The high efficient work flow and standards of pre-hospital rescuecreated in this study saved pre-hospital rescue time thus is also a benefit for hospital rescue.It would be of help in loweringthe mortality and disability rate of severe trauma.
