摘要
目的探讨改进型交锁髓内钉(Ⅱ型)闭合复位内固定治疗股骨干骨折的疗效并总结经验。方法我院骨科2014年7月至2016年7月共收治的股骨干骨折患者67例,其中采用改进型交锁髓内钉(Ⅱ型)内固定术治疗患者28例(交锁髓内钉内固定组),采用传统切开复位钢板内固定术治疗患者39例(普通钢板固定组),对比及分析两组治疗方法的手术情况及术后恢复情况和治疗经验。结果接受改进型交锁髓内钉内固定术治疗患者的临床效果要明显好于传统的钢板内固定,具体表现在前者的术中出血量、术后疼痛严重程度及持续时间、术后并发症发生率、骨折愈合时间明显少于后者(所有P<0.05)。交锁髓内钉内固定组中的平均手术时间长于对照组(P<0.05)。结论对于普通的股骨干骨折,采用传统的普通钢板内固定的手术方法应尽量少用;改进型交锁髓内钉(Ⅱ型)固定治疗股骨干骨折符合快速康复原则。
Objective To summarize the experience of the closed reduction/interlocking intramedullary nails(Ⅱ type)in treatment of femoral fractures. Methods A total of 67 cases were treated in our hospital from July 2014 to July 2016,in whom 28 cases were performed the surgical approach of closed reduction and interlocking intramedullary nails(Ⅱtype)fixation(observation group),39 underwent traditional open reduction and plate internal fixation(control group). The clinical data were collected and analyzed and our experience was summarized. Results The patients received closed reduction and interlocking intramedullary nails(Ⅱtype)fixation showed better efficacy which were less intraoperative blood loss,smaller postoperative complications,shorter fracture healing time when compared with those undergoing traditional open reduction and plate internal fixation(all P values less than 0.05). However,there was longer operative time in observation group than that in control(P < 0.05). Conclusion For femoral fractures,the traditional method of internal fixation with ordinary steel plate should be used as little as possible. interlocking intramedullary nails(Ⅱ type)and internal fixation in the treatment of femoral fractures conform to the principle offast track surgery.
