摘要
目的 :探讨肝巨片形吸虫病患者腹部CT影像学特点、诊断价值及对比治疗后腹部CT病变吸收。方法:回顾分析2011年3月至2012年3月爆发的27例肝巨片形吸虫病例的临床表现、检验结果及腹部CT表现,并对比2例肝巨片形吸虫感染患者治疗好转后的腹部CT病变吸收。结果:肝巨片形吸虫病临床表现无明显特异性,ELISA检测巨片形吸虫抗体阳性。27例CT平扫均显示为肝肿大并实质内弥漫斑片状低密度影,边界不清晰,可相互融合,2例复查腹部CT结果提示经治疗好转后1~2年内低密度影可见吸收,但改变不明显,4年后复查结果提示腹部CT恢复至正常。结论:肝巨片形吸虫感染后腹部CT有一定的临床特点,结合临床表现及抗体检测可提高诊断率,治疗好转后较长时间腹部CT可基本恢复正常。
Objective:To investigate the radiological features and diagnostic value of abdominal CT in the patients with fasciola hepatica and to compare abdominal CT lesions absorption before and after the treatment.Methods:The study was a retrospective analysis of the medical records of27patients with hepatic Fasciola gigantica in The First Affiliated Hospital of Dali University during the period from Mar.2011to Mar.2012,including the clinical features,laboratory results and abdominal CT findings.A comparative analysis was also conducted on the abdominal CT lesions absorption of2patients after treatment.Results:The clinical manifestations of fasciola hepatica showed no obvious specificity and ELISA detection of Fasciola gigantica was antibody positive.27cases of CT scan showed hepatomegaly and parenchymal diffuse patchy low density,and the boundary was not clear but combined.The abdominal CT findings of2patients after treatment showed that after1year to2years of recovery low density shadow displayed a visible absorption,but the change was not obvious.The review of the results after4years suggested that abdominal CT was restored to the normal level.Conclusion:The abdominal CT has certain clinical features after the liver is largely infected.The combination of clinical manifestations with antibody detection can improve the diagnosis rate;abdominal CT can basically return to a normal level after a longer time of recovery.
