摘要
目的比较锁定钢板内固定与外固定支架治疗老年性桡骨远端C1型骨折的临床疗效。方法选择2012年7月至2015年12月安徽省立医院收治的桡骨远端C1型老年骨折患者80例为研究对象,根据随机数字表法分为观察组和对照组各40例。对照组患者接受掌侧锁定钢板内固定治疗,观察组患者接受外固定支架治疗,记录两组患者的术中出血量、手术时间、总住院时间;术后6个月随访,记录骨折复位效果、并发症发生情况。结果观察组患者的术中出血量、手术时间、总住院时间分别为(45.38±5.91)mL、(23.28±2.94)min和(6.27±0.84)d均少于对照组的(76.19±8.34)mL、(60.15±6.84)min和(10.63±1.86)d,组间差异有统计学意义(t=19.063,31.321,13.511,P<0.05);治疗6个月后,观察组与对照组患者的腕关节功能和骨折复位相关指标比较,差异无统计学意义(P>0.05);两组患者的并发症发生率差异无统计学意义(P>0.05)。结论外固定支架治疗老年性桡骨远端C1型骨折的手术效果与内固定治疗相似,但是前者可减少术中患者出血量,缩短手术和住院时间,具有一定临床应用优势。
ObjectiveTo compare the clinical effects of locking plate internal fixation with external fixator in the treatment of thoseelderly patients with C1-type distal radius fractures.MethodsA total of80elderly patients with C1-type distal radius fractures,ever treatedin our hospital from Jul2012to Dec2015,were chosen as the study objectsandthen were divided into the study group and the control group in accordance with random number table,40patients in each group.Patients in the control group underwentinternal fixation with volar locking plates,while those in the study group were treated with external fixators,and after operation all patients were followed up for6months.The intraoperative bleeding volume,operation timeand hospitalization time,together with state of fracture reduction and complicationsduringthe follow-upperiod,were recordedand comparedbetween thetwo groups.ResultsThe intraoperative bleeding volume,operation time and total hospitalization time in the study group were(45.38±5.91)mL,(23.28±2.94)min and(6.27±0.84)d,respectively,which were significantly less than thosein the control group[(76.19±8.34)mL,(60.15±6.84)min and(10.63±1.86)d;allP<0.05].There was no significant difference in the state ofwrist jointfunction and fracture reduction at6monthsafter operationbetween the two groups(P>0.05),so was difference inthe incidence of complications between them(P>0.05).ConclusionApplication of external fixator in the treatment of elderly patients withC1-type distal radius fracturesshows similar clinical effectsto locking plate internal fixation,but it could reduce the patients'intraoperative blood loss andshorten theiroperation and hospitalizationtime,with certain advantages inclinical application.
