4D CTA与DSA在烟雾病分级中的对比研究 被引量:7

摘要 目的利用4D CTA对烟雾病的血管改变进行Suzuki分级,以DSA为"金标准",评价无创血管成像在烟雾病的血管评估中的应用价值。方法符合入选标准的烟雾病患者101例均行4D CTA及DSA检查,两者检查时间间隔小于1周。由2名神经放射学专家对4D CTA(VR结合MIP)及DSA图像共同进行评估,以DSA作为金标准,参照DSA图像的改良法Suzuki分级,对4D CTA评估烟雾病的血管改变进行相应的评分。将4D CTA与DSA各评分进行一致性检验,并计算4D CTA的Kappa值及其95%可信区间。另外将4D CTA与DSA各评分进行相关性分析。结果 4D CTA与DSA评估烟雾病血管的改良法Suzuki分级的一致性评分Kappa值为0.714(0.649~0.778),相关系数及P值为0.843(0.000)。结论4D CTA对烟雾病的Suzuki分级与DSA具有较强的一致性与相关性,能够无创的作为临床上烟雾病患者严重程度评估的主要手段。 Objective To evaluate noninvasive4D dynamic volume computed tomography angiography(CTA)compared to digital subtraction angiography(DSA)in vascular assessment of adult moyamoya disease using the modified Suzuki scoring system.Methods101patients underwent4D CTA and DSA within1week interval between both examinations.The images were evaluated by2neuroradiologists using the modified Suzuki scoring system.Consistency test with calculation ofκvalue and95%confidence interval as well as correlation analysis was performed on the scores of4D CTA and DSA.ResultsThe4D CTA and DSA scores were consistent(κ=0.714,95%confidence interval:0.649-0.778)with significant(P=0)correlation coefficient of0.843.Conclusion4D CTA is comparable to DSA in assessing the severity of moyamoya disease.
