摘要
目的探讨局部进展期结直肠癌侵犯小肠的临床病理特征及受侵犯小肠区域淋巴结转移情况。方法回顾性分析汕头大学医学院第一附属医院自2016年1月至2017年4月收治的7例局部进展期结直肠癌侵犯小肠的病例资料。结果 7例结直肠癌联合小肠切除者均无致死性并发症发生。术后病理证实小肠受癌性侵犯者5例,4例受侵犯小肠引流区域淋巴结阳性,淋巴结阳性率为80%。结论结直肠癌联合小肠切除是安全可行的,术中应同时清扫受侵犯小肠引流区域淋巴结。
Objective To explore the clinicopathological characteristics and lymph node metastasis of locally advanced colorectal carcinoma(LACRC) with small bowel invasion. Methods The data of seven patients with LACRC involving small bowel treated in The First Affiliated Hospital Of Shantou University Medical College from March 2016 to April 2017 were analyzed retrospectively. Results All patients were treated with multivisceral resection including a standard radical colorectal resection plus involved small bowel removal. without lethal complication. Pathological examination confirmed tumor invasion of the small bowel in71%(5/7) cases, of which 80%(4/5) has positive small bowl mesenteric lymph node metastasis. Conclusions Multivisceral resection is safe to perform on patients with LACRC involving small bowel. Moreover,the authors suggest that the mesenteric lymph nodes of the involvedsmall bowel should be dissected to achieve radical resection.
