摘要
目的:评价CT引导下经皮穿刺碘125(^(125)I)粒子植入术对晚期肝细胞癌的治疗效果。方法:采用前瞻性病例对照研究方法,选取接受肝动脉栓塞化疗的32例晚期肝癌患者,采用数表法将其随机分为治疗组和对照组,每组16例。治疗组在接受肝动脉栓塞化疗的基础上接受经CT引导下^(125)I粒子植入术,对照组仅接受肝动脉栓塞化疗。治疗过程中记录患者的不良反应,随访3个月后评价两组间疗效的差异,以肿瘤的影像学变化为依据评价治疗的有效率。结果:治疗组治疗有效率为75.00%,显著高于对照组的31.25%,两组比较差异有统计学意义(x^2=8.981,P<0.05);两组患者治疗不良反应之比差异无统计学意义(x^2=0.59,P>0.05)。结论:CT引导经皮穿刺^(125)I粒子植入术可显著提高接受肝动脉栓塞化疗的晚期肝癌患者的治疗有效率,且不增加并发症。
Objective:To evaluate the curative effect of the implanted125I particle of percutaneous puncture under guidance of CT for advanced hepatocellular carcinoma.Methods:A prospective case-control study was implemented in the research,and32patients with advanced hepatocellular carcinoma who underwent transcatheter hepatic arterial chemoembolization were divided into treatment group(16cases)and control group(16cases)as random number table.The patients of treatment group received implantation of125I particle of percutaneous puncture under the guidance of CT on the basis of transcatheter hepatic arterial chemoembolization,while patients of control group only received transcatheter hepatic arterial chemoembolization.The adverse reactions of patients in the therapeutic process were recorded,and the differences of curative effect between two groups were compared and evaluated after followup of3months.Besides,the iconography changes of carcinoma were used as the basis of evaluating effective rate of treatment.Results:The effective rate of treatment group(75.00%)was significantly higher than that of control group(31.25%)(x2=8.981,P<0.05),while there was no significant difference about adverse reaction between the two groups(x2=0.59,P>0.05).Conclusion:The implantation of125I particle of percutaneous puncture under the guidance of CT can significantly increase effective rate of treatment for patients with advanced hepatocellular carcinoma who received transcatheter hepatic arterial chemoembolization,and it don't increase the complication and other adverse reactions.
