摘要
目的构建老年人慢性病多维健康测定量表的高血压模块。方法参考高血压患者生命质量测定量表(QLICD-HY)构建老年人慢性病多维健康测定量表高血压模块的初始问卷,包括临床症状、用药副作用、社会和心理问题3个侧面共13个条目(HY1~HY13)。于2012年,选取在东莞市石龙博爱医院、东莞市大朗人民医院就诊且明确诊断为高血压的老年患者62例进行问卷调查。根据患者问卷得分,采用变异程度、相关性分析、因子分析、Cronbach'sα系数进行统计学筛选,最后结合专家意见进行量表条目筛选。结果统计学分析结果显示,HY2、HY5、HY6、HY11、HY12、HY13符合得分变异程度<0.90、与量表得分相关系数<0.50、因子分析中因子载荷<0.50、删除后模块总Cronbach'sα系数升高中的1项或以上,考虑删除。结合专家意见,最终形成的老年人慢性病多维健康测定量表高血压模块共包括8个条目,重新编号后分别为:HTN1头晕头痛,HTN2耳鸣,HTN3心慌气促,HTN4下肢或脚腕部位水肿,HTN5肢体无力、麻木或活动不灵,HTN6胸口痛或肩背痛,HTN7视力减退或视物模糊,HTN8饮食限制或生活方式改变。结论根据严格程序所得到的8个条目的老年人慢性病多维健康测定量表高血压模块具有较好的代表性和内容效度。
Objective To develop the Hypertension Scale in Multidimensional Health Measurement Instrument System for Elderly Patients with Chronic Disease(MHIEC-HY).Methods Based on the Quality of Life Instruments for Chronic Diseases-Hypertension Scale(QLICD-HY),we developed the initial 13-item MHIEC-HY consisting of 3 dimensions(clinical symptoms,drug side effects,social and psychological issues)and using it to survey 62 elderly patients with hypertension diagnosed confirmedly in Dongguan Shilong Bo-Ai Hospital and Dongguan Dalang People's Hospital in 2012.The items were selected again based on the pilot survey results analysis by using variability method,correlation coefficient method,factor analysis,and Cronbach'sαand results of Delphic expert consultation.Results Six items(2,5,6,11,12,13)were deleted due to the following reasons:variability coefficients of items 5,6,12 were below 0.90;correlation coefficients of items 2,5,6,11,13 with the scale score were below 0.50;factor loadings of these 5 items were below 0.50;the Cronbach'sαof the corresponding dimension was elevated after deleted item 13.Based on the results of the data analyses and expert consultation,an 8-item specific scale for elderly patients with hypertension was formed,its reordered items were:HTN1(headache or dizzy),HTN2(tinnitus),HTN3(heart palpitations or shortness of breath),HTN4(swelling in ankle or legs),HTN5(weakness,numbness or inactivity in limbs),HTN6(chest pain or shoulder and back pain),HTN7(loss of vision or blurred vision),HTN8(dietary restriction or life style change).Conclusion The 8-item scale developed by the strict procedures is a good representation for hypertension measurement scales and it has satisfactory content validity.
