摘要
目的了解成人血流感染金黄色葡萄球菌的分布及耐药性。方法收集本院血流感染金黄色葡萄球菌患者的临床资料,对其进行分析。结果 193株金黄色葡萄球菌中,社区获得性血流感染甲氧西林敏感金黄色葡萄球菌(CA-MSSA)共37株,主要分布在骨科、急诊及内分泌科;医院获得性血流感染甲氧西林敏感金黄色葡萄球菌(HA-MSSA)共86株,主要分布在肾脏内科及ICU;医院获得性血流感染甲氧西林耐药金黄色葡萄球菌(HA-MRSA)共70株,主要分布在ICU、肾脏内科,总体上SA菌株分布以肾脏内科、ICU为主。MRSA对红霉素、克林霉素、环丙沙星、左氧氟沙星、四环素、庆大霉素、复方新诺明、克林霉素、莫西沙星的耐药率比HA-MSSA高,CA-MSSA与HA-MSSA的抗生素耐药性无显著差异。结论肾脏内科、ICU是金黄色葡萄球菌血流感染的主要分布科室,临床医师应根据科室及药敏结果变更选择适当的抗生素。
Objective To investigate the clinical distribution and drug resistance of staphylococcus aureus bacteremia in adults with blood infection.MethodsThe clinical data of the patients with Staphylococcus aureus bacteremia in our hospital were analyzed.Results Out of 193 Staphylococcus aureus,37 strains were community acquired methicillin sensitive Staphylococcus aureus in bloodstream infections(CA-MSSA),mainly distributed in departments of orthopaedics,emergency and endocrinology,86 strains were methicillin sensitive Staphylococcus aureus(HA-MSSA)in hospital-acquired bloodstream infections,mainly distributed in the department of kidney and ICU,70 strains were methicillin resistant staphylococcus aureus(HA-MRSA)in hospital-acquired bloodstream infections,mainly distributed in the ICU,kidney department.As a whole,SA strain was distributed in kidney department,and ICU.MRSA had higher resistance to erythromycin,clindamycin,ciprofloxacin,levofloxacin,tetracycline,gentamycin,and compound sulfamethoxazole,clindamycin,moxifloxacin HA-MSSA.There were no differences in antibiotic resistance between HA-MSSA and CA-MSSA.Conclusion The renal department and ICU are main distributed departments for staphylococcus aureus infection.The physician or pharmacist should select appropriate antibiotics according to clinical departments and drug sensitivity.
