Treg细胞及其细胞因子IL-10在阻塞性睡眠呼吸暂停低通气综合征合并急性脑梗死患者外周血中的动态表达研究 被引量:6

摘要 目的:观察Treg细胞及其细胞因子IL-10在阻塞性睡眠呼吸暂停低通气综合征(OSAHS)合并急性脑梗死(ACI)患者外周血中的动态表达,分析其与神经功能缺损程度之间的关系,探讨OSAHS对急性脑梗死的影响。方法:选取ACI患者160例,依据多导睡眠监测结果分为观察组(OSAHS合并ACI患者)86例,对照1组(单纯ACI患者)74例,发病后24h及7d、14d进行NIHSS评分,同时抽取静脉血,另选健康体检者80例作为对照2组,抽取空腹静脉血1次,分别采用流式细胞术和酶联免疫吸附试验(ELISA)检测三组各时期Treg细胞在外周血中所占的百分率,以及细胞因子IL-10的表达水平。结果:ACI发病后24h、7d及14d,观察组及对照1组的Treg细胞百分率及IL-10水平均显著低于对照2组,差异有统计学意义,且均随时间呈逐渐上升趋势,各时间点差异显著,观察组各时间点Treg细胞百分率及IL-10水平均显著低于对照1组,差异有统计学意义(P均<0.05)。ACI发病后24h、7d及14d,观察组及对照1组的NIHSS评分均随时间呈逐渐下降趋势,观察组NIHSS评分各时间点均显著高于对照1组,差异有统计学意义(P均<0.05)。结论:OSAHS合并ACI患者外周血Treg细胞百分率及其细胞因子IL-10的表达水平较单纯ACI患者显著降低,呈动态变化过程,且与神经功能缺损程度可能相关。 Objective:To observe the dynamic changes of Treg cells and interleukin-10(IL-10)in peripheral blood of patients with acute cerebral infarction and OSAHS,analyze their relationship with neurological impairment,and investigate the effect of OSAHS to ACI.Methods 160 patients with ACI were selected and divided into two groups according to PSG results.86 patients with ACI and OSAHS were enrolled in research group,and 74 patients with only ACI were enrolled in control group 1.NIHSS score was used to evaluate the neurological deficits at 24h,day 7 and day 14 after symptom onset.3ml whole blood of each patient was collected at the same time.Flow cytometry and enzyme-linked emmunosorbent assay(ELISA)were used to detected the percentage of Treg cells and IL-10 level in peripheral blood.80 healthy people underwent physical examination were selected as control group 2.The 3ml whole blood was also collected,and the percentage of Treg cells and the IL-10 level were also detected.Results:1.The percentage of Treg cells and the IL-10 level of research group and control group1 were significantly lower than control group2 at 24h,7days and 14days after acute cerebral infarction.The trend was gradual rised with time.The results at each time spot were significantly different.The percentage of Treg cells and IL-10 level of research group were significantly lower than control group1(P<0.05).2.The NIHSS scores of research group and control group 1 were gradually decreased from 24h,7days to 14 days after acute cerebral infarction.The NIHSS score of research group was higher than that of control group at each time spot.The difference was significant(P<0.05).Conclusion:The percentage of Treg cells and IL-10 level of patients with OSAHS and ACI are significantly lower than patients with only ACI.The results changed dynamically.Those were relative with the level of neurological impairment.
