关节镜下高强度线捆扎固定后交叉韧带胫骨止点撕脱骨折 被引量:4

Arthroscopic Fixation with High Strength Line for Posterior Cruciate Ligament Tibial Avulsion Fracture

摘要 目的:探讨膝关节镜下应用高强度线捆扎固定后交叉韧带(posterior cruciate ligament,PCL)胫骨止点撕脱骨折的疗效。方法:选取2014年6月-2016年12月本院在膝关节镜辅助下应用高强度线治疗PCL胫骨止点撕脱骨折手术患者12例,关节镜监视下对骨折块进行复位,PCL胫骨隧道导向器钻取骨隧道,高强度线捆扎骨块,螺钉及锚钉固定。术后对患者进行随访,观察手术效果。结果:术后随访6个月~2年,平均9个月,术后复查X线片示骨折块复位满意,术后3个月复查X线片示骨折愈合,关节面平整。末次随访检查,患者后抽屉试验检查弱阳性1例,其余患者均为阴性;患者膝关节活动度与健侧对比均无差别,均未出现关节僵硬、关节不稳及关节感染;Lysholm膝关节功能评分为(93.2±2.3)分。结论:关节镜监视下复位PCL胫骨止点撕脱骨折并使用高强度线捆扎固定骨折块,具有创伤小,恢复快的优点,方法可靠易行,是一种值得推广的手术方式。 Objective:To investigate the effect of knee arthroscopy in treatment of posterior cruciate ligament(PCL)tibial avulsion fracture with high strength line.Method:A total of 12 patients with PCL tibial avulsion fracture were treated with high strength line under the knee arthroscope from June 2014 to December 2016 were selected.The fracture blocks were reset under arthroscopy,the PCL tibial tunnel guide was drilled through the bone tunnel,the high strength line was tied up,the hollow nail and the anchor were fixed.After the operation,they were followed up to observe the effect of the operation.Result:The postoperative follow-up was 6 months-2 years,with an average of 9 months.After the operation,the X-ray showed that the fracture block was satisfactory,and the X-ray showed that the fracture healed and the articular surface was smooth 3 months after the operation.After the last follow-up examination,1 case were weak positive in the posterior drawer test,and the other patients were negative,there was no difference between the knee joint activity and the contralateral side,and there was no joint stiffness,joint instability and joint infection,the score of Lysholm knee joint function was(93.2±2.3)scores.Conclusion:Arthroscopic reduction of the PCL tibial avulsion fracture and the use of high strength line strapping fixed fracture,has the advantages of small trauma and quick recovery,is reliable and easy to operate,which is a kind of operation method worthy of popularization.
