摘要
为了观察308 nm准分子激光照射联合311 nm窄谱中波紫外线(311NB-UVB)对白癜风自体表皮移植术后及预后的影响,对自体表皮移植术治疗白癜风患者,分别术后2周实施308 nm准分子激光治疗或/和311NB-UVB照射治疗,观察照射后起效时间、皮片痊愈率、预后及生存质量(DLQI)、不良反应发生率及复发率。结果显示,与单一308 nm准分子激光治疗比较,308 nm准分子激光联合311NB-UVB照射治疗起效时间显著短,皮片痊愈率高,不良反应总发生率及1年随访复发率低,差异显著(P均<0.05);两种照射均可明显降低患者皮肤组织肿瘤坏死因子(TNF-α)、DLQI评分,升高碱性成纤维细胞生长因子(bFGF),且联合照射的效果较单一308 nm准分子激光更显著。提示308 nm准分子激光联合311NB-UVB较单一308 nm准分子激光照射疗法能显著提高白癜风自体表皮移植术后患者疗效、预后。
In order to learn the effects of 308 nm excimer laser irradiation combined with 311 nm narrow-band ultraviolet(311NB-UVB)on the prognosis of patients with vitiligo after autologous epidermal grafting,eighty patients with vitiligo treated with autologous epidermal grafting were treated with 308 nm excimer laser and/or 311NB-UVB irradiation after surgery in 2 weeks.The onset time,skin graft healing rate,tumor necrosis factorα(TNF-α),basic fibroblast growth factor(bFGF),quality of life(DLQI),the incidence rates of adverse reactions and recurrence rates were observed.Compared with the single treatment of 308 nm excimer laser,the treatment of the308 nm excimer laser and 311NB-UVB irradiation have shorter onset time,higher skin graft healing rate,lower total incidence rate of adverse reactions and recurrence rate in 1-year follow-up(even P<0.05).The TNF-αlevels and DLQI scores were significantly decreased while bFGF levels were significantly increased after irradiation,and the changes in the combined irradiation were more significant than those in the single irradiation(P<0.05),suggesting that 308 nm excimer laser combined with 311NB-UVB irradiation has more advantages over 308 nm excimer laser single irradiation therapy for patients with vitiligo after autologous epidermal grafting in terms of efficacy.
