摘要
目的:调查上海社区慢性病老年患者的延续性照护需求现状并探讨护理对策。方法:调查200例社区老年慢性病患者的日常生活自理能力、一般情况、延续性照护需求,探讨相关照护对策。结果:未签约家庭医生、患有多种慢性病、月收入较低、医疗保险为自费等是患者有延续性需求的主要因素。大多数患者具有多方面的延续性照护需求,需采取相关对策给予患者服务。结论:社区慢性病老年患者对延续性照护的需求较高,政府和医院应创建延续性照护服务平台。
Objective:To investigate the status quo of the continuous care demand of the elderly patients with chronic diseases in Shanghai community and to discuss the nursing countermeasures.Methods:To investigate the daily life self-care ability,general situation and continuous care demand of 200 cases of elderly patients with chronic diseases in the community,and to discuss the related care countermeasures.Results:Unsigned family doctors,suffering from a variety of chronic diseases,low monthly income,and medical insurance were the main factors for the patient's continuous care demand.Most patients had multiple continuous care demand and need to take relevant countermeasures to provide service to the patients.Conclusion:The elderly patients with chronic disease in the community were in high demand for continuous care.The government and hospital should create a continuous care service platform.
