摘要
目的探讨无创正压通气治疗慢性阻塞性肺疾病急性加重的临床疗效及安全性,从而为临床治疗COPD急性加重期提供有效的参考依据。方法 2016年1月~2017年2月期间我院160例慢性阻塞性肺疾病急性加重期患者,随机分为对照组(鼻导管低流量通气治疗)和观察组(无创正压通气治疗),比较两组治疗的临床疗效,治疗前后血气分析指标及肺功能指标变化情况,以及住院时间及病死率。结果与对照组比较,观察组治疗的总有效率明显增高,差异有统计学意义(P<0.05);与对照组比较,观察组治疗后PH、Pa O2、Pa CO2均有明显改善,FEV1/FVC、FEV1、MVV均得到显著提高,差异有统计学意义(P<0.05);与对照组相比,观察组住院时间及病死率均显著降低,差异有统计学意义(P<0.05)。结论无创正压通气治疗能够有效改善慢性阻塞性肺疾病急性加重期患者的血气分析结果及肺功能,明显提高治疗的临床效果及安全性。
Objective To study the clinical efficacy and safety of noninvasive positive pressure ventilation(NIPPV)in the treatment of acute exacerbation of chronic obstructive pulmonary disease(COPD),and provide the reference for the clinical treatment of acute exacerbation of COPD.Methods From January 2016 to February 2017,160 patients with acute exacerbation of chronic obstructive pulmonary disease in our hospital were randomly divided into control group(low flow nasal catheter ventilation)and observation group(NIPPV treatment).The clinical efficacy of treatment,changes of blood gas index and lung function before and after treatment,the hospitalization time and mortality rate in two groups were observed and compared.Results Compared with the control group,the total effective rate of treatment was significantly increased in the observation group with statistical difference;Compared with the control group,the blood gas index such as PH,PaO2,PaCO2 were obviously improved and the lung function including FEV1/FVC,FEV1,MVV were also increased on after treatment in the observation group with statistical difference;Compared with the control group,the time of hospitalization and the mortality of the observation group were significantly lower,and the difference was statistically significant.Conclusion The NIPPV treatment can effectively improve the blood gas index and lung function in patients with acute exacerbation of chronic obstructive pulmonary disease,and significantly improve the clinical efficacy and treatment safety.
