摘要
目的探讨亚临床甲状腺功能减退症与心踝血管指数、脉搏波传导速度及超敏C反应蛋白之间的相关性。方法随机选取所在医院2015年5月—2017年5月住院及门诊患者80例(入选患者均除外合并冠心病、糖尿病、脑卒中、感染、严重肝肾功能不全、肿瘤),根据甲状腺功能进行分组,亚临床甲减患者40例为观察组(仅TSH升高,<10μIU/m L,T3、T4、FT3、FT4均在正常范围),甲状腺功能正常者40例为对照组,比较两组患者的心踝血管指数、脉搏波传导速度及超敏C反应蛋白并进行统计分析。结果观察组患者右心-踝血管指数(R-CAVI)(9.14±1.003),左心-踝血管指数(L-CAVI)(9.14±1.028),颈-股脉搏波传导速度(C-FPWV)(9.99±1.370)m/s,超敏C反应蛋白(hs-CRP)(3.41±1.001)mg/L。对照组右心-踝血管指数(R-CAVI)(8.51±0.954),左心-踝血管指数(L-CAVI)(8.60±0.944),颈-股脉搏波传导速度(C-FPWV)(9.18±0.812)m/s,超敏C反应蛋白(hs-CRP)(2.61±1.171)mg/L。观察组较对照组上述各项指标均升高,差异有统计学意义(P<0.05)。结论亚临床甲减患者的R-CAVI、L-CAVI、C-FPWV及hs-CRP均高于对照组,亚临床甲状腺功能减退症患者较甲状腺功能正常患者动脉硬化风险增加,心血管疾病的发病风险升高。
Objective This paper tries to investigate the correlation between subclinical hypothyroidism and cardio-ankle vascular index,pulse wave velocity and hypersensitive C-reactive protein.Methods A total of 80 patients(including hospital patients and outpatients)from May 2015 to May 2017(all the patients were excluded of coronary heart disease,diabetes,stroke,infection,severe liver and renal insufficiency,tumour)were selected and divided into two groups,40 of them complicated with subclinical hypothyroidism were in the observation group(only TSH raised,less than 10μIU/mL,T3,T4,FT3,FT4 were in normal range),40 of them with normal euthyroidism were in control group.Cardio-ankle vascular index,pulse wave conduction velocity and hypersensitive C-reactive protein of the two groups were compared and analyzed.Results The right cardio-ankle vascular index(R-CAVI)were[(9.14±1.003)vs(8.51±0.954)],left cardio-ankle vascular index(L-CAVI)were[(9.14±1.028)vs(8.60±0.944)],carotid-femoral pulse wave velocity(C-FPWV)were[(9.99±1.370)m/s vs(9.18±0.812)m/s],hypersensitive C-reactive protein(hs-CRP)(mg/L)were[(3.41±1.001)vs(2.61±1.171)]in the observation group and in the control group respectively.The index were higher in the observation group than in the control group,and the difference was significant(P<0.05).Conclusion The R-CAVI,L-CAVI,C-FPWV and hs-CRP were higher than those in the control group with subclinical hypothyroidism,patients with subclinical hypothyroidism had more increased risk of ateriosclerosis than the patients with euthyroidism with increased risk of cardiovascular disease.
