摘要
目的研究腹腔镜下碘-125(^(125)I)放射性粒子治疗不可切除胰腺癌患者的临床效果。方法将我院临床诊断为不可切除胰腺癌的75例患者根据治疗方法分为三组,每组25例。A组不做放化疗(患者自愿选择),B组给予吉西他滨+白蛋白结合型紫杉醇正规化疗;C组采用腹腔镜下联合B超^(125)I植入粒子。比较三组患者的临床效果。结果术后1个月,B组与C组有效率明显高于A组,C组高于B组(P<0.05)。治疗后7、14、30 d,B组及C组患者的VAS评分明显低于A组,C组低于B组(P<0.05)。治疗后,B组和C组患者SF-36评分明显提高,C组患者SF-36评分高于A组、B组(P<0.05)。B组、C组中位生存期长于A组,但差异不显著(P>0.05)。结论对于不可切除胰腺癌患者,行腹腔镜下联合B超125I放射性粒子植入,近期疗效确切,患者疼痛症状可得到有效缓解,生活质量明显提高,生存期有所延长。
Obsjective To investigate the clinical effect of laparoscopic iodine-125(125I)radioactive particle implantation in patients with unresectable pancreatic carcinoma.Methods A total of 75 patients diagnosed as unresectable pancreatic carcinoma in our hoapital were divided into three groups according to the treatment methods,with 25 cases in each group.Gourp A was treated with non-chemoradiotherapy(following patients'willingness),group B was treated with chemotherapy of gemcitabine and white protein binding taxol,and group C was treated wih laparoscopic and ultrasonic B 125I radioactive particle implantation.The clinical effects were compared among the three groups.Results One month after operation,the effective rates in the group B and C were significantly higher than that in group A,and that in the group C was higher than the group B(P<0.05).At 7、14、30 d after treatment,the VAS scores of patients in group B and C were significantly lower than those in group A,and those in the group C were lower than the group B(P<0.05).After treatment,the SF-36 scores in group B and C significantly increased,and those in the group C were higher than the group A and B(P<0.05).The median survival time of group B and C were longer than that of group A,but the differences were not significant(P>0.05).Conclusion Laparoscopic 125I radioactive particle implantation could improve the short-term curative effect,pain relief effect,life quality and survival curve in patients with unresectable pancreatic carcinoma.
