摘要
目的探讨降钙素原、超敏C反应蛋白联合白介素6动态检测在新生儿感染早期诊断中的价值。方法方便选取该院2015年2月—2017年2月期间新生儿科收治的125例感染性疾病新生儿为观察组,其中75例为细菌感染组,50例为病毒感染组,细菌感染组中重症感染35例,轻症感染40例,并以同期该院收治的50例非感染性疾病新生儿为对照组研究对象。所有新生儿均检测血清降钙素原(PCT)、超敏C反应蛋白(hs-CRP)及白介素6(IL-6)。观察各组新生儿指标检测结果,并以工作特征(ROC)曲线分析3种指标的疾病诊断特异性。结果细菌感染组患儿PCT、hs-CRP、IL-6各指标均显著高于病毒感染组和对照组新生儿,差异有统计学意义(P<0.05)。重症感染组PCT、hs-CRP、IL-6水平明显高于轻症感染组,轻症感染组的3项指标明显高于对照组(P<0.05)。PCT、hs-CRP、IL-6 3项指标的ROC曲线下面积分别为85.33、70.12、82.24,灵敏度为74.67%、70.67%、80.00%,特异性为90.67%、66.67%、68.00%。结论PCT、hs-CRP、IL-6均能作为判定新生儿早期感染的指标之一,但白介素6的灵敏度更高,降钙素原的特异性更好,联合动态检测可以提高诊断的灵敏度和特异性。
Objective This paper tries to investigate the value of procalcitonin,high sensitivity C-reactive protein combined with interleukin-6 in the early diagnosis of neonatal infection.Methods Convenient selected a total of 125 neonates with infectious diseases were enrolled in this hospital from February 2015 to February 2017.Of these,75 were bacterial infection,50 were virus infection,and in the group 35 cases of infection,40 cases of mild infection,and the same period our hospital treated 50 cases of non-infectious diseases of newborns as the control group of subjects.Serum levels of procalcitonin(PCT),hypersensitive C-reactive protein(hs-CRP)and interleukin-6(IL-6)were measured in all newborns.The results of each group of neonates were observed,and the disease specificity of the three indexes was analyzed by the working characteristic(ROC)curve.Results The levels of PCT,hs-CRP and IL-6 in children with bacterial infection were significantly higher than those in virus infection group and control group(P<0.05).The levels of PCT,hs-CRP and IL-6 in the severe infection group were significantly higher than those in the mild infection group(P<0.05).The three indexes of the mild infection group were significantly higher than those of the control group,the ROC curve of PCT,hs-CRP and IL-6 the specific area was 85.33,70.12,82.24,the sensitivity was 74.67%,70.67%,80.00%,the specificity was 90.67%,66.67%,68.00%.Conclusion PCT,hs-CRP and IL-6 can be used as one of the indicators to determine the early infection of neonates.However,the sensitivity of interleukin 6 is higher,the specificity of procalcitonin is better,and the combined dynamic detection can improve the sensitivity of diagnosis and specificity.
