摘要
目的探讨一期与分期输尿管软镜碎石术(FURL)治疗上尿路结石的临床疗效。方法回顾性分析2016年1月至2017年3月该院收治的105例行FURL患者的临床资料,其中一期组47例,除2例因输尿管狭窄置鞘困难和1例结石梗阻以上尿液明显浑浊而留置双J管二期手术外,其余44例患者均成功碎石;分期组58例,均成功碎石。比较两组成功碎石患者手术时间、置鞘成功率、结石清除率、并发症发生率、总住院时间的差异。结果两组患者手术时间、置鞘成功率、结石清除率、并发症发生率比较,差异均无统计学意义(P>0.05)。一期组患者总住院时间[(5.39±1.50)d]明显短于分期组[(6.60±1.38)d],差异有统计学意义(P<0.05)。结论在严格把握手术适应证的条件下,一期和分期FURL在有效性和安全性方面相当,但一期FURL治疗周期和住院时间更短、治疗费用更低,可在临床开展应用。
Objective To investigate the clinical efficacy of one-stage and staged flexible ureteroscope lithotripsy(FURL)in the treatment of upper urinary tract calculi.Methods The clinical data of 105 patients underwent FURL in this hospital from January 2016 to March 2017 were retrospectively analyzed,including 47 cases of one-stage group,excepting 2 cases adopted double-J tube indwelling because of difficult sheath placing due to ureterostenosis and 1 case of obvious urine cloudy,other 44 cases were succeeded in lithotripsy;and 58 cases of staged group all succeeded in lithotripsy.Then the operation time,placing sheath success rate,stone clearance rate,complication occurrence rate,total hospitalization time were compared between the two groups.Results The operation time,placing sheath success rate,stone clearance rate and complication occu-rrence rate had no statistical difference between the two groups(P>0.05).The total hospitalization time in the one-stage group was(5.39±1.50)d,which was significantly shorter than(6.60±1.38)d in the staged group,the difference was statistically signifi-cant(P<0.05).Conclusion Under the condition of strictly grasping the indications of operation,the one-stage FURL is comparable to the staged FURL in terms of efficacy and safety,moreover has a shorter treatment period,shorter hospital stay and lower cost,which can be carried and applied in clinic.
