Xpert MTB/RIF在抗酸染色阳性痰标本结核分枝杆菌及利福平耐药性检测中的应用分析 被引量:4

摘要 目的:探讨多色巢式实时荧光定量核酸扩增检测技术(Multicolor nested real-time fluorescence quantitative nucleic acid amplification detection technology,Xpert MTB/RIF)在抗酸染色阳性痰标本结核分枝杆菌及利福平耐药性检测中的应用价值。方法:选取我院2015年11月—2017年10月疑似肺结核患者190例,进行Xpert MTB/RIF检测、比例法药敏试验、改良罗氏固体培养、显微镜检查。结果:以改良罗氏固体培养法检测结果作金标准,Xpert MTB/RIF检测敏感度为90.00%(81/90)、特异度为89.00%(89/100),两者具有较高一致性(K=0.789);以痰涂片染色法检测结果作金标准,Xpert MTB/RIF检测敏感度为86.67%(91/105)、特异度为98.82%(84/85),两者具有较高一致性(K=0.843);以比例法药敏试验结果作金标准,Xpert MTB/RIF检测利福平耐药敏感度为83.33%(10/12)、特异度为100.00%(67/67),两者具有较高一致性(K=0.895)。结论:采取多色巢式实时荧光定量核酸扩增检测技术可迅速诊断抗酸染色阳性痰标本结核分枝杆菌,并能准确检测利福平耐药性,为临床制定有效干预方案提供可靠依据。 Objective:To investigate the application value of multicolor nested real-time fluorescence quantitative nucleic acid amplification detection technology in the detection of mycobacterium tuberculosis and Rifampicin resistance in anti acid-stained positive sputum specimens.Methods:A total of 190 cases of suspected pulmonary tuberculosis in our hospital from November 2015 to October 2017 were selected.Xpert MTB/RIF test,proportional drug sensitivity test,improved Roche solid culture and microscopy examination were performed.Results:The modified Roche's solid culture method was used as the gold standard.The sensitivity of Xpert MTB/RIF detection was 90%(81/90)and the specificity was 89%(89/100),and they had high consistency(K=0.789).The sputum smear staining method was used as the gold standard.The sensitivity of Xpert MTB/RIF detection was 86.67%(91/105),and the specificity was 98.82%(84/85).The two groups had high consistency(K=0.843).Taking the results of the proportional drug susceptibility test as the gold standard,the sensitivity of rifampicin to Xpert MTB/RIF was 83.33%(10/12)and the specificity was 100%(67/67),and they had high consistency(K=0.895).Conclusion:Multicolor nested real-time fluorescence quantitative nucleic acid amplification detection technology can quickly diagnose the Mycobacterium tuberculosis in positive sputum specimens,and can accurately detect rifampicin resistance,so as to provide a reliable basis for formulating effective intervention programs.
