摘要
目的探讨多囊卵巢综合征(PCOS)患者血清抗苗勒氏管激素(AMH)水平与胰岛素抵抗和雄激素水平的相关性。方法选择2015年10月—2017年10月收治的PCOS 150例作为观察组,另选择同期卵巢功能正常者70例。检测2组AMH水平、糖代谢指标及性激素水平,并分析AMH水平与胰岛素抵抗和雄激素水平的相关性。结果观察组体重指数、AMH、黄体化激素(LH)、空腹血糖(FPG)、胰岛素抵抗指数(HOMA-IR)、雄烯二酮(A)、脱氢表雄酮(DHEA)水平均高于对照组(P<0.05)。PCOS患者AMH水平与雌二醇(E_2)水平呈负相关,与T、A、DHEA、HOMAIR呈正相关(P<0.05),与BMI、卵泡刺激素(FSH)、LH、FPG无明显相关性(P>0.05)。结论 PCOS患者血清AMH水平升高,且与胰岛素抵抗和雄激素水平呈正相关,可能与PCOS患者糖代谢紊乱和激素分泌异常有关。
Objective To study correlation between serum anti-mullerian hormone(AMH)level with insulin resistance and androgen in patients with polycystic ovary syndrome(PCOS).Methods A total of 150 PCOS patients admitted during October 2015 and October 2017 were selected as observation group,while 70 people with normal ovarian function at the same period were selected as control group.AMH,glycometabolism and sex hormone levels were detected,and correlations between AMH level with insulin resistance and androgen were analyzed in two groups.Results Values of body mass index,AMH,luteinizing hormone(LH),fasting plasma glucose(FPG),homeostasis model of assessment-insulin resistance(HOMA-IR),5-androstenedione(A)and dehydroepiandrosterone(DHEA)levels in observation group were significantly higher than those in control group(P<0.05).In PCOS patients,AMH level was negatively correlated with estradiol(E 2)level,and positively correlated with testesterone(T),A,DHEA and HOMA-IR(P<0.05);there were no significant correlations between the level with BMI and follicle stimulating hormone(FSH),LH and FPG(P<0.05).Conclusion Serum AMH level in PCOS patients may decrease,and it is positively correlated with insulin resistance and androgen,which may be related to disorder of glucose metabolism and abnormal secretion of hormone.
