摘要
目的观察骨形态发生蛋白-7(BMP-7)复合纤维蛋白对大鼠骨质疏松性椎体骨折愈合的效果。方法2014年3月—2016年8月在新疆医科大学动物实验中心进行实验,选取清洁级雌性SD大鼠72只,随机数字表法分为实验组、模型组、假手术组各24只,实验组和模型组均去除卵巢建立骨质疏松模型,假手术组仅切除卵巢周围部分软组织;3个月后均建立骨折模型,实验组给予BMP-7 24μg+纤维蛋白20μg凝胶状混合物处理骨折部位,模型组和假手术组于手术部位放入等体积的小牛血清。术后观察3组术后骨组织形成情况、椎体力学相关指标、椎体骨组织学形态参数。结果 (1)术后第4周,实验组和假手术组的L5椎体骨折处可见大量软骨细胞并与纤维连接呈网状,模型组的L5椎体骨折处可见大量软骨细胞并与纤维少量连接呈网状。术后第8周,实验组和假手术组骨性骨痂已经基本形成,纤维骨痂消失;模型组的骨痂形成较为疏松,存在大量的纤维性骨痂。(2)术后第6、8、12周,模型组大鼠的最大载荷、最大应变量、弹性模量测定值均低于同期的假手术组、实验组,差异均具有统计学意义(F=24.513、6.087、20.396,P=0.000、0.019、0.000),假手术组、实验组对比差异无统计学意义(t=1.354、0.861、1.176,P=0.078、0.124、0.095)。(3)术后第12周,模型组大鼠的TBV%、MTPT测定值均低于同期的假手术组和实验组(F=4.327、23.671,P=0.033、0.000),TRS%、MAR测定值高于假手术组、实验组(F=4.589、2.079,P=0.031、0.046)。实验组和假手术组对比差异无统计学意义(t=0.643、0.652、1.431、0.038,P=0.165、0.161、0.074、1.049)。结论局部应用BMP-7复合纤维蛋白对大鼠骨质疏松性椎体骨折愈合具有显著的促进作用。
bjective To investigate the effect of bone morphogenetic protein 7(BMP-7)fibrin complex on osteoporotic vertebral fracture healing.Methods The experiment was done in animal experimental center of Xinjiang Medical University from March 2017 to August 2017,72 female SD rats of clean grade were selected and randomly divided into experimental group,model group and sham operation group;24 rats in each group.Both the experimental group and the model group were ovariectomized to establish the osteoporotic model,while the sham operated group was only excised some soft tissue around the ovary;After 3 months,three groups of rats were established fracture model,the experimental group was treated with 24μg BMP-7+fibrin 20μg mixture to treat the fracture site,and the model group and the sham operation group were put into equal volume calf serum at the operation site.After operation,HE staining was used to observe the bone formation in the three groups;the vertebral mechanics related indexes of three groups were tested;the morphological parameters of vertebral body were measured in three groups of rats.Results (1)At fourth weeks after operation,the L 5 vertebral fracture of the experimental group and sham operation group showed a large number of cartilage cells and reticular fiber connection,and the L 5 vertebral fracture of the model group showed a large number of cartilage cells and reticular fiber small connection.At eighth weeks after operation,both in the experimental group and the sham operation group had bony callus formed,and the fibrous callus disappeared.The callus formation in the model group was relatively loose,and there were a lot of fibrous callus.(2)At sixth,eighth,twelfth weeks after operation:the maximum load,the maximum strain,and the elastic modulus values of the model group were lower than those at the same period in the sham operation group and the experimental group,the differences were statistically significant(F=24.513,F=6.087,F=20.396,P=0.000,P=0.019,P=0.000);There was no statistical difference between the sham operation group and the experimental group(F=1.354,F=0.861,F=1.176,P=0.078,P=0.124,P=0.095).(3)At twelfth weeks after operation:TBV%and MTPT of rats in model group were lower than those at the same period in the sham operated group and the experimental group(F=4.589,F=2.079,P=0.031,P=0.046),TRS%and MAR were higher than those in the sham group and the experimental group(F=4.589,F=2.079,P=0.031,P=0.046).There was no significant difference between the experimental group and the sham operation group(F=0.643,F=0.652,F=1.431,F=0.038,P=0.165,P=0.161,P=0.074,P=1.049).Conclusion Focal application of BMP 7 fibrin complex has significant promoting effect on the healing of osteoporotic vertebral fracture in rats.
