摘要
目的探讨结直肠癌患者血清CEA、CA199的表达,及其与结直肠癌肝转移的关系。方法回顾性分析我院收治的180例结直肠癌患者,将其分为肝转移组和非肝转移组,检测两组患者血清CEA、CA199的表达变化,分析二者的表达与结直肠癌肝转移的关系。结果血清CEA、CA199阳性的结直肠癌患者比例分别为62.8%及48.3%,二者的表达与患者的年龄、性别、病程、肿瘤的大小、肿瘤的部位无明显相关(P>0.05),而与肿瘤的浸润深度、分化程度显著相关(P<0.05);同时发现CEA及CA199的表达与结直肠癌肝转移具有明显的统计学相关性(P<0.01),并且联合检测血清CEA及CA199的表达对于结直肠癌肝转移的预测价值较高;联合检测CEA与CA199的表达对于预测结直肠癌肝转移患者的生存期更有价值(P<0.05);多因素分析提示血清CA199、肿瘤分化程度、淋巴结分期是患者总生存期的独立预后因素。结论结直肠癌患者血清CEA、CA199表达变化与肝转移明显相关。
Objective To explore the expression of serum CEA and CA199 in colorectal cancer and analyze the association between the expression of CEA and CA199 and colorectal cancer liver metastasis.Methods 180 cases with colorectal cancer of our hospital were analysed retrospectively,which were divided into liver metastasis group and non-liver metastasis group.The expression of serum CEA and CA199 was detected,and the association was explored between the expression of CEA and CA199 and the colorectal cancer liver metastasis.Results The positive rates of serum CEA and CA199 expression were 62.8%and 48.3%respectively.The expression of CEA and CA199 was relevant with infiltration depth and differentiation degree(P<0.05),but it was irrelevant with age,sex,tumor size,duration,and location(P>0.05).The expressions of CEA and CA199 were statistically associated with liver metastasis(P<0.05),and combining detection of serum CEA and CA199 had statistical significance in predicting colorectal cancer liver metastasis.The survival analysis indicated that combining detection of serum CEA and CA199 had statistical significance in predicting overall survival of the patients with colorectal cancer liver metastasis(P<0.05).Multiple factor analysis revealed that serum CA199,lymphnode stage and differentiation degree were independent prognosis factors for overall survival.Conclusion The expression of serum CEA and CA199 in colorectal cancer patients was significantly associated with liver metastasis.
