摘要
目的探讨孕期糖化血红蛋白水平检查在诊断妊娠期糖尿病(GDM)中的效果分析。方法选取2016年1月~2017年10月在我院产科门诊行妊娠期糖尿病筛查的患者100例,根据75g葡萄糖耐量(OGTT)试验结果分成正常组(NGR组)和糖尿病组(GDM组),对比两组患者糖化血红蛋白(HbA1C)水平与OGTT水平,并分析HbA1C在诊断妊娠期糖尿病中的阳性检出率。结果 GDM组患者OGTT试验2小时血糖水平(10.52±4.03)mmol/L明显高于正常值,也明显高于NGR组(6.61±0.94)mmol/L,Hb A1C水平也明显高于NGR组,经t检验(P<0.05)。HbA1C阳性者在GDM患者中阳性检出率达87.5%,经χ~2检验,差异无统计学意义(P>0.05),说明Hb A1C检查在筛查GDM中与OGTT试验无统计学意义。而且HbAlC水平越高,并发症发生率逐渐提高。结论孕期糖化血红蛋白水平检查在诊断妊娠期糖尿病中有极高的灵敏度,且操作简便,值得临床进一步推广应用。
Objective To explore the effect of glycosylated hemoglobin level during pregnancy on diagnosis of gestational diabetes mellitus(GDM).Methods From January 2016 to October 2017,100 patients with gestational diabetes screening were selected and divided into normal group(group NGR)and diabetes group(group GDM)according to the results of 75g glucose tolerance test(OGTT).The levels of glycosylated hemoglobin(HbA1C)and OGTT in the two groups were compared,and the positive rate of HbA1C in the diagnosis of gestational diabetes mellitus was analyzed.Results In group GDM,the level of 2 hour blood glucose was(10.52±4.03)mmol/L and it was significantly higher than that of normal value in OGTT test.It was also significantly higher than that of group NGR which was(6.61±0.94)mmol/L,and the HbA1C level was significantly higher than that of NGR group,and(P<0.05)by t test.The positive rate of HbA1C positive in GDM patients was 87.5%,and(P>0.05)byχ2 test,indicating that there was no significant difference between HbA1C examination and OGTT test in GDM screening.Moreover,the higher the level of HbAlc,the higher the incidence of complications.Conclusion The level of glycosylated hemoglobin in pregnancy is very sensitive in the diagnosis of gestational diabetes mellitus,and it is easy to operate.It is worthy of further clinical application.
