摘要
目的采用携白细胞介素(IL)-8单克隆抗体(IL-8单抗)的靶向对比剂行心肌声学造影(MCE)定量评价兔心肌梗死的心肌微循环灌注。材料与方法选取健康成年日本大耳白兔30只,通过抽签的方式随机分为急性心肌梗死组和假手术组,每组15只。制备兔心肌梗死模型,将模型分别于术前、术后30 min、2 h及6 h行常规心脏超声检查、Sono Vue对比剂及携IL-8单抗靶向对比剂MCE检查。应用QLAB 8.0软件,定量分析左心室短轴乳头肌水平各节段心肌充盈影像。结果 Sono Vue对比剂及携IL-8单抗的靶向对比剂MCE检查发现,术后心肌梗死组造模节段前壁、前间隔的2种对比剂视频强度超声均值均低于非梗死节段、术前及假手术组(P均<0.05);携IL-8单抗靶向对比剂MCE前壁、前间隔的对比剂超声均值均高于Sono Vue对比剂对应节段,达峰时间、开始显影时间均早于Sono Vue对比剂对应节段(P均<0.05)。结论靶向对比剂携带的IL-8单抗对缺血心肌释放的炎性因子IL-8存在靶向作用。缺血性心脏病可通过携IL-8单抗靶向对比剂MCE检查进行更早、更精确的诊断。
Purpose To quantitatively evaluate the myocardial microcirculation perfusion in rabbit myocardial infarction by myocardial contrast echocardiography(MCE)with IL-8 monoclonal antibody carried target contrast agent.Materials and Methods Thirty healthy adult Japanese white rabbits were randomly assigned into two groups:acute myocardial infarction group and sham operation group(15 in each group).Rabbit myocardial infarction models were successfully established.Routine echocardiography,MCE with SonoVue contrast agent and IL-8 monoclonal antibody carried target contrast agent were respectively performed before surgery and at 30 minutes,2 hours and 6 hours after surgery.QLAB 8.0 software was applied to quantitatively analyze the myocardial filling images of each segment of the left ventricular short axis papillary muscle.Results MCE results with SonoVue contrast agent and IL-8 monoclonal antibody carried target contrast agent found that the video intensity average value of the two agents on antetheca and anteroseptum segments in the myocardial infarction group after operation were lower than that on non-infarction segments,in preoperative and sham operation group(P<0.05);for MCE with IL-8 monoclonal antibody carried target contrast agent,the video intensity average value on antetheca and anteroseptum segments were higher,the peak time and development time were earlier than those with SonoVue contrast agent(P<0.05).Conclusion The IL-8 monoclonal antibody carried by target contrast agent has a targeting effect on the inflammatory factor IL-8 released by ischemic myocardium.Ischemic heart disease can be detected earlier and more accurately by MCE examination with IL-8 monoclonal antibody carried target contrast agent.
