摘要
目的探讨实时超声弹性成像对于B超未发现足底筋膜炎筋膜形态学改变的诊断价值。资料与方法纳入36例足底筋膜炎患者(42只足)作为研究组,并选择24例健康志愿者(48只足)作为对照组。采用B超图像测量筋膜最大厚度;采用实时超声弹性成像图像对筋膜进行弹性图像评分;并计算相对周围软组织的应变率。比较各种方法的诊断价值。结果研究组筋膜厚度、弹性图像评分、应变率均高于对照组,差异有统计学意义(P<0.001)。B超中筋膜厚度测量法的曲线下面积、敏感度、特异度分别为0.841、64.3%、91.7%,应变比法的曲线下面积、敏感度、特异度分别为0.906、81.0%、83.3%,实时超声弹性图像中评分法的曲线下面积、敏感度、特异度分别为0.861、85.7%、72.9%。结论 RTE可显示足底筋膜炎的筋膜弹性减低,且较B超具有更高的敏感性。
Purpose To investigate the value of real-time elastosonography(RTE)in the diagnosis of plantar fasciitis in patients whose morphological changes in the fascia could not be detected by B-mode ultrasound.Materials and Methods A total of 36 patients with plantar fasciitis(42 feet)and 24 healthy volunteers(48 feet)were enrolled in this study.The maximum thickness of fascia was measured by B-mode ultrasound.The fascia was scored according to corresponding RTE image and the strain rate relative to surrounding soft tissue was calculated.The value of these methods in the diagnosis of plantar fasciitis was compared.Results The fascia thickness,the score of elastic image and the strain ratio in the patient group were higher than those in the control group,with statistically significant differences(P<0.001).The area under the curve,sensitivity,specificity of fascial thickness measurement method using B-mode ultrasound were 0.841,64.3%and 91.7%,respectively;while they were respectively 0.906,81%and 83.3%for strain rate method,and were respectively 0.861,85.7%and 72.9%for elasticity scoring method using RTE.Conclusion RTE can reveal the decrease of fascial elasticity in plantar fasciitis,with higher sensitivity than B-mode ultrasound.
