三维斑点追踪成像技术对心肌梗死患者左室功能的评价 被引量:5

Sensitivity and specificity of three-dimensional speckle tracking imaging in the diagnosis of left ventricular function in patients with myocardial infarction

摘要 目的探讨三维斑点追踪成像技术对心肌梗死患者左室功能的评价。方法选取2016年1月至2017年5月期间入院就诊的心肌梗死患者46例,设病例组,并选取同期体检健康者40例作为对照组,两组均行三维斑点追踪成像检查,测量整体纵向应变(GLS)、整体径向应变(GRS)、整体圆周应变(GCS)及整体面积应变(GAS)各应变量值,并分析各应变量用于诊断心肌梗死敏感度和特异度。结果观察组左心室舒张末期容积、收缩末期容积均明显较对照组高,且观察组左心室射血分数明显较对照组低,组间统计差异有统计学意义(P <0.05)。观察组患者GLS、GRS、GCS及GAS各应变量值均较对照组低,组间统计差异有统计学意义(均P <0.05)。经受试者工作特征曲线(ROC)分析,GLS=9.23时,ROC曲线下的面积(AUC)为9.69,此时诊断心肌梗死敏感度和特异度分别为91.22%、71.06%;GRS=28.46时,AUC为8.20,此时敏感度和特异度分别为74.01%、71.01%;GCS=13.25时,AUC为7.68,敏感度和特异度分别为68.95%、76.92%;GAS=21.04时,AUC为8.98,敏感度和特异度分别为85.36%、93.33%。结论 3D-STI技术能够确切监测左心室收缩功能各阶段心肌应变值,其各应变值用于诊断心肌梗死铭感度和特异度均较高,为心肌梗死临床诊断提供新思路。 Objective To investigate the sensitivity and specificity of three-dimensional speckle tracking imaging(3D-STI)in the diagnosis of left ventricular function in patients with myocardial infarction.Methods Forty-six patients with myocardial infarction admitted to our hospital from January 2016 to May 2017 were enrolled into the case group,and 40 healthy people were enrolled into the control group.Patients in two groups were examined by three-dimensional speckle tracking imaging.The corresponding variables of GLS,GRS,GCS and GAS were measured,and the sensitivity and specificity of each index were analyzed for the diagnosis of myocardial infarction.Results The left ventricular end diastolic volume and the end systolic volume in the observation group were higher than those in the control group,while the left ventricular ejection fraction in the observation group was significantly lower than that in the control group(P<0.05).The values of GLS,GRS,GCS and GAS in the observation group were significantly lower than those in the control group(P<0.05).Through ROC analysis,AUC was 9.69 when GLS was 9.23,the sensitivity and specificity of diagnosis of myocardial infarction were 91.22%and 71.06%,respectively.AUC was 9.69 when GLS was 9.23,the sensitivity and specificity of diagnosis of myocardial infarction were 91.22%and 71.06%,respectively.AUC was 8.20 when GLS was 28.46,the sensitivity and specificity of diagnosis of myocardial infarction were 74.01%and 71.01%,respectively.AUC was 7.68 when GCS was 13.25,the sensitivity and specificity of diagnosis were 68.95%and 76.92%.AUC was 8.98 when GAS was 21.04,the sensitivity and specificity of diagnosis were 85.36%and 93.33%,respectively.Conclusion 3D-STI technique can accurately monitor the myocardial left ventricular strain at various stages of the strain value for the diagnosis of myocardial infarction with higher sensitivity and specificity,providing a new idea for the clinical diagnosis of myocardial infarction.
