摘要
目的探讨二尖瓣综合成形术(COMVAR)治疗风湿性二尖瓣病变的临床疗效。方法选择2015年9月至2017年8月在郑州大学第二附属医院行二尖瓣手术的60例风湿性二尖瓣病变患者为研究对象,以行二尖瓣综合成形术的患者为瓣膜成形组(n=30),行二尖瓣置换术的患者为瓣膜置换组(n=30)。对比两组患者心功能、心脏彩超、围手术期临床指标、并发症和随访情况。结果术后3个月,两组患者心功能分级差异无统计学意义(P> 0.05);心脏彩超情况较治疗前均有改善(P <0.05),且瓣膜成形组优于瓣膜置换组(P <0.05)。两组患者围手术期临床指标及并发症差异无统计学意义(P> 0.05)。瓣膜成形组短期随访情况优于瓣膜置换组(P <0.05)。结论二尖瓣综合成形术(COMVAR)治疗风湿性二尖瓣病变的临床疗效显著。
Objective To explore the curative effect of comprehensive mitral valve apparatus reconstruc-tion(COMVAR)on rheumatoid mitral lesions.Methods 60 patients with rheumatic mitral valve disease were divided into mitral valve repair group and mitral valve replacement group.The patients in the mitral valve repair group were treated with COMVAR,and those in the mitral valve replacement group with mitral valve replacement.The postoperative cardiac function classification,echocardiography,perioperative clinical indicators,complica-tions,and follow-up results were compared between the two groups.Results Three months after surgery,there was no significant difference in cardiac function between the two groups(P>0.05).The cardiac ultrasound of the two groups were better than before the treatment(P<0.05),and the cardiac ultrasound of the mitral valve repair group was better than that of the mitral valve replacement group(P<0.05).There was no significant difference between the two groups in perioperative clinical indicators and complications(P>0.05).The short-term follow-up effect of the mitral valve repair group was better than that of the mitral valve replacement group(P<0.05).Conclusion The clinical efficacy of the COMVAR in treatment of rheumatic mitral lesions is significant.
