颞骨肿瘤动态增强MRI定量参数与肿瘤定性的初步研究 被引量:7

Dynamic Contrast-Enhanced MRI of Temporal Bone Tumors: A Preliminary Study of Correlation between Quantitative Parameters and Malignancy

摘要 目的探讨动态增强磁共振成像(dynamic contrast-enhanced magnetic resonance imaging,DCE-MRI)定量参数在鉴别颞骨良恶性肿瘤的作用。方法收集2012年1月至2018年7月初诊且行颞骨MRI平扫及动态增强检查的颞骨肿瘤患者21例,均行手术及病理证实,其中颞骨恶性肿瘤13例,颞骨良性肿瘤或肿瘤样病变8例。在3.0TMR动态增强扫描图手绘感兴趣区(region of interest,ROI),测量各定量参数值:容量转移常数(volume transfer con-stant,Ktrans)、速率常数(rate constant,Kep)、血管外细胞外间隙容积分数(extravascular extracellular volume fraction,Ve)和初始曲线下面积(initial Area under curve,iAUC)。采用独立样本t检验,比较颞骨恶性肿瘤组和良性病变组间各定量参数的差异,绘制受试者工作特征曲线(receiver operating characteristic curve,ROC),分析各定量参数值对颞骨良、恶性肿瘤的鉴别诊断效能。结果颞骨恶性肿瘤组和良性肿瘤或肿瘤样病变组的年龄,性别无统计学差别。所有的病变均有明显强化或不均匀明显强化。恶性组和良性的Ktrans值分别为0.382±0.153(min-1),0.228±0.096(min-1),两组之间差异具有显著统计学差别(t=2.543, P=0.020);两组的Kep值分别为0.552±0.221(min-1),0.277±0.072(min-1),两组间值差别有统计学意义(t=3.378, P=0.003)。两组间的Ve,iAUC无显著统计学差别(P>0.05)。结论 DCE-MRI在鉴别颞骨肿物良恶性中具有很重要的价值,尤其是Ktrans和Kep。 Objective To evaluate the relationship between parameters acquired by dynamic contrast-enhanced magnetic resonance imaging(DCE-MRI)and malignant temporal bone tumors.Methods From January 2012 to July 2018,21 patients with temporal bone tumors who underwent plain and dynamic contrast-enhanced MRI were enrolled.Diagnoses were confirmed by surgery and pathology.Among them,13 had malignant tumors and 8 had benign tumors or tumor-like lesions.The region of interest(ROI)was drawn by hand on 3.0T MR dynamic contrast-enhanced scans,and the following parameters were quantified:volume transfer constant(Ktrans),rate constant(Kep),extravascular extracellular volume fraction(Ve),and apparent diffusion coefficient(ADC).Using independent sample t test,these parameters were compared between malignant and benign tumors.The receiver operating characteristic(ROC)curve was used to determine the differential diagnostic efficacy of the parameters for benign as well as malignant tumors.Results There was no significant age differences between patients with malignant tumor and those with benign tumors or tumor-like lesions.All lesions were markedly enhanced or heterogeneous.Ktrans was 0.382±0.153(min-1)for malignant tumors and 0.228±0.096(min-1)for benign lesions(t=2.543;P=0.020).Kep was 0.552±0.221(min-1)and 0.277±0.072(min-1)for malignant and benign lesions,respectively(t=3.378;P=0.003).Ve and iAUC were significantly different between malignant and benign lesions(P>0.05).Conclusion DCE-MRI parameters,specifically Ktrans and Kep,can be used for differentiating benign and malignant tumors of the temporal bone.
