摘要
目的评估血尿酸水平对原发性高血压合并冠心病病变程度的影响。方法选取本院2017年1月至2017年12月行冠状动脉造影的原发性高血压患者208例为研究对象,将单纯原发性高血压患者(103例)纳入单纯高血压组,原发性高血压合并冠心病患者(105例)纳入合并冠心病组。收集患者的一般资料并行体格检查及实验室检查。按照血尿酸水平四分位数法将原发性高血压合并冠心病患者分为四组:第1四分位数组(0μmol/L <血尿酸≤291μmol/L,28例),第2四分位数组(291μmol/L <血尿酸≤355μmol/L,26例),第3四分位数组(355μmol/L <血尿酸≤422.5μmol/L,27例),第4四分位数组(血尿酸> 422.5μmol/L,24例)。分析单纯高血压组和合并冠心病组患者的血尿酸水平,以及血尿酸水平与原发性高血压合并冠心病之间的关系。结果合并冠心病组患者的血尿酸水平显著高于单纯高血压组(P <0.05),且随着血尿酸水平的升高,左心室收缩末期容积、左心室舒张末期容积、左心室舒张末内径均显著增大(P_均<0.05),左心室射血分数显著降低(P <0.05)。结论高血压合并冠心病患者的血尿酸水平与冠心病病变程度密切相关。
Objective To evaluate the effect of serum uric acid level on the severity of essential hypertension complicated with coronary artery heart disease.Method From January 2017 to December 2017,all 208 patients who underwent coronary angiography were selected as the subjects.103 cases of patients with essential hypertension were enrolled into hypertension group,and 105 cases of patients with essential hypertension complicated with coronary artery heart disease were enrolled into essential hypertension complicated with coronary artery heart disease group.General information were collected,and physical examination laboratory testing were analyzed and compared.Patients with essential hypertension complicated with coronary artery heart disease were divided into four groups according to the quartile of serum uric acid level:the first quartile group(0μmol/L<serum uric acid≤291μmol/L,n=28),the second quartile group(291μmol/L<serum uric acid≤355μmol/L,n=26),the third quartile group(355μmol/L<serum uric acid≤422.5μmol/L,n=27),the fourth quartile group(serum uric acid>422.5μmol/L,n=24).To analyze the relationship between serum uric acid level in essential hypertension group and essential hypertension complicated with coronary artery heart disease group,and the relationship between serum uric acid level and essential hypertension complicated with coronary artery heart disease.Result The serum uric acid level in patients with essential hypertension complicated with coronary artery heart disease group was significantly higher than that in essential hypertension group(P<0.05).With the increase of serum uric acid level,the left ventricular end-systolic volume,left ventricular end-diastolic volume and left ventricular end-diastolic diameter significantly increased(Pall<0.05),and the left ventricular ejection fraction significantly decreased(P<0.05).Conclusion The increase of serum uric acid level in patients with hypertension complicated with coronary artery heart disease is closely related to the degree of coronary artery heart disease.
