摘要
目的探讨彩色多普勒超声血流参数在评估肠系膜上动脉重度狭窄中的价值。方法回顾性分析经CTA证实的肠系膜上动脉重度狭窄患者超声资料,记录肠系膜上动脉起始段狭窄处舒张期流速(EDV)、收缩期峰值流速(PSV)、与肠系膜上动脉开口水平腹主动脉收缩期流速之比(PSV/PSV_(AO)),与狭窄远端收缩期流速之比(PSV/PSV_(distal))等血流参数。应用SPSS 22.0软件绘制ROC曲线,分析上述各血流参数的诊断价值。结果彩色多普勒超声共检出18例阳性,12例阴性。CTA检查证实超声结果中假阳性3例,假阴性2例。彩色多普勒超声敏感性88%,特异性77%。ROC曲线表明:在诊断肠系膜上动脉重度狭窄的参数中,EDV和PSV/PSV_(distal)的诊断价值最高,其ROC曲线下面积>0.90。以EDV=58 cm/s为诊断界值时,诊断狭窄的灵敏度和特异度分别为100%和77%;以PSV/PSV_(distal)=2.79为诊断界值时,诊断狭窄的灵敏度和特异度分别为94%和77%。结论彩色多普勒超声血流参数对肠系膜上动脉重度狭窄有较高的诊断价值,其中EDV和PSV/PSV_(distal)这两个血流参数的诊断价值最高。
Objective To approach the color Doppler flow imaging(CDFI)parameters in the evaluation of severe superior mesenteric artery stenosis.Methods We retrospectively analyzed the CDFI parameters of patients with severely superior mesenteric artery stenosis confirmed by CTA.All the CDFI parameters including EDV,PSV,PSV/PSVAO,PSV/PSVdistal were recorded.Software SPSS was used to draw a ROC curve,and diagnostic reliability of each CDFI parameter was evaluated.Results 18 positive cases and 12 negative cases were revealed by CDFI,and 3 falsepositive and 2 false-negative were determined by CTA.the sensitivity of CDFI was 88%,specificity,77%.ROC curves revealed:EDV and ratio of PSV/PSVdistal were firmly related with severely superior mesenteric artery stenosis,the area uder curve was exceed 0.90.When EDV was equal to 58 cm/s,the sensitivity,100%,the specificity,77%.When PSV/PSVdistal was 2.79,the sensitivity,94%,the specificity,77%.Conclusion CDFI parameters are of great value in diagnosis of severe superior mesenteric artery stenosis,both the parameters including EDV and PSV/PSVdistal are most valuable for the diagnosis.
