摘要 目的探讨MUC1和趋化因子CXCL16在乳腺癌诊断和治疗监测中的应用。方法采用FQ-PCR检测50例健康女性体检者、102例良性乳腺肿瘤、157例乳腺癌和126例其他肿瘤组外周血中MUC1 mRNA,ELISA检测CXCL16,分析与乳腺癌患者临床组织学分期的关系,以及手术前后的差异。结果乳腺癌组MUC1和CXCL16均高于正常对照组和良性乳腺疾病组,MUC1高于其他肿瘤组,而CXCL16则无显著性差异; MUC1和CXCL16在手术后有显著下降;在乳腺癌组中,随着临床分期的升高,MUC1和CXCL16递增,Ⅲ+Ⅳ期显著高于I+Ⅱ期; MUC1和CXCL16联合检测可提高灵敏度、阳性预测值和阴性预测值。结论 MUC1和CXCL16对乳腺癌预后有一定的预测作用,MUC1和CXCL16对术后随访有意义,联合检测有助于提高对乳腺癌早期诊断。 To investigate the clinical application of MUC1 gene expression and CXCL16 in the diagnosis and therapy of breast cancer.The mRNA expressions of MUC1 in 50 health women,102 patients with benign breast disease,157 patients with malign breast disease and 126 patients with other malignant tumors were detected by FQ-PCR.CXCL16 was detected by ELISA.The relationships between the two indexes and the clinicopathological characteristics of breast cancer patients including histological grade and before and after surgery were analyzed.The expressions of MUC1 and CXCL16 in the breast cancer group were higher than those in normal controls and benign breast disease group.MUC1 in the breast cancer group were higher than those in the other malignant tumors group.There were no significant difference in CXCL16 between the breast cancer group and the other malignant tumors group.The expressions of MUC1 and CXCL16 were decreased after operation.In the breast cancer group,the expressions of MUC1 and CXCL16 were increased with clinical stages,that is,their expressions were higher in the stage III+IV than stage I+II.The specificity of MUC1 was higher than CXCL16.The sensitivity of CXCL16 was higher than MUC1.The sensitivity and positive and negative predictive values were improved by the combined of CXCL16 and MUC1.MUC1 and CXCL16 play a role to predict the prognosis of breast cancer.MUC1 and CXCL16 can follow-up the curative effect of breast cancer after operation.The joint detection of MUC1 and CXCL16 is helpful to improve the early diagnosis of breast cancer.
