摘要
目的探讨绝经后女性骨代谢标志物与冠状动脉病变程度的相关性。方法回顾性选择2016年2月至2018年2月同济大学附属杨浦医院收治的绝经后女性冠心病住院患者共124例,采用Gensini积分判断冠状动脉病变的严重程度,并分两组(Gensini积分≥60分组48例和Gensini <60分组76例)。测定Ⅰ型胶原交联C端肽(CTX)、甲状旁腺激素(PTH)和骨钙素水平,分析上述骨代谢指标与冠状动脉病变程度指标Gensini积分的关系。结果 Gensini积分≥60分组患者的血清CTX水平显著低于Gensini积分<60分组(0. 35±0. 06 ng/ml vs. 0. 47±0. 07 ng/ml,t=5. 285,P=0. 002),两组患者血清PTH和骨钙素水平差异无统计学意义(t=0. 978,1. 509; P=0. 271,0. 101)。Spearman相关分析发现,只有CTX与Gensini积分存在较好的负相关性(r=-0. 651,P <0. 001)。结论绝经后女性冠心病患者血清CTX水平降低与冠状动脉病变程度存在明显的负相关性。
Objective To explore the relationship between markers of bone metabolism and degree of coronary artery diseases in postmenopausal women.Methods A total of 124 postmenopausal women with coronary artery disease admitted in this hospital during February 2016 to February 2018 were retrospectively studied.The Gensini scores were applied to judge the severity of lesions of coronary artery,they were devided into two groups:48 cases with Gensini scores≥60 group and 76 cases in Gensini scores<60 group..The relationship between levels of type Ⅰcollagen cross-linking C-terminal peptide(CTX),parathyroid hormone(PTH)and osteocalcin were measured,and the relationship between above-mentioned bone metabolic markers and the degree of coronary artery disease had been analyzed.Results There was significant statistical difference in serum CTX levels between these two groups(0.35±0.06 vs.0.47±0.07 ng/ml,t=5.285,P=0.002).Serum CTX levels in Gensini scores≥60 group were significantly lower than those with Gensini scores<60 group,and there was no significant difference in serum levels of PTH and osteocalcin between these two groups(t=0.978,1.509;P=0.271,0.101).Spearman correlation analysis found that only CTX had good negative correlation with Gensini scores(r=-0.651,P<0.001).Conclusion The decrease of serum level of CTX in postmenopausal women with coronary heart disease has negative correlation with the degree of coronary artery disease.
