主动运动疗法治疗膝关节骨性关节炎的临床观察及对X线下生物力学指标的影响 被引量:3

摘要 目的 观察临床应用主动运动疗法治疗膝关节骨性关节炎(KOA)的临床效果及对X线下生物力学指标的影响。方法 采用前瞻性研究方法,选取2016年9月至2018年5月在新疆医科大学第五附属医院诊治的60例膝关节骨性关节炎患者,按照随机平行分组法随机分为对照组和观察组,每组各30例。对照组患者应用超声波治疗,观察组给予超声波+主动运动疗法治疗。对比分析两组患者治疗后的疼痛程度(VAS评分)、平衡功能(BBS评分)及X线下膝关节生物力学指标。结果 治疗后两组患者的VAS评分(4.62±1.24、2.15±0.63)均较治疗前(8.14±2.41、8.17±2.33)明显降低(P<0.05),BBS评分(31.61±7.26、42.35±5.47)明显升高(P<0.05),观察组患者各指标评分变化幅度明显高于对照组(P<0.05);治疗后两组患者的股骨角、胫骨角以及关节间隙角均有明显改善(P<0.05),观察组患者各指标改善程度明显高于对照组(P<0.05);两组患者治疗后患足最大负重(33.32±17.36 kg vs.65.47±25.52 kg)以及负重比例数值差(0.46±0.22%vs.1.71±0.34%)比较,观察组明显高于对照组(P<0.05)。结论 临床应用主动运动疗法治疗KOA,可明显改善症状,促进患者平衡稳定,是治疗KOA的有效方法。 Objective To observe the clinical effect of the clinical application of active motion therapy in the treatment of knee osteoarthritis(KOA)and its impact on the biomechanics of the X-ray.Methods A prospective study was conducted.60 patients with knee osteoarthritis who were treated in the Fifth Affiliated Hospital of Xinjiang Medical University from September 2016 to May 2018 were randomly divided into control group and observation group with 30 cases in each group.The patients in the control group were treated with ultrasonic treatment,and the observation group was treated with ultrasound plus active exercise therapy.The degree of pain(VAS score),balance function(BBS score)and biomechanical index of knee joint under X-ray were compared and analyzed between the two groups.Results After treatment,the VAS scores(4.62±1.24,2.15±0.63)of the two groups were significantly lower than those before treatment(8.14±2.41,8.17±2.33)(P<0.05),the BBS scores(31.61±7.26,42.35±5.47)were significantly higher(P<0.05),and the changes of each index scores in the observation group were significantly higher than those in the control group(P<0.05).The femoral angle,tibial angle and joint clearance angle of the two groups were significantly improved(P<0.05),and the improvement degree of each index in the observation group was significantly higher than that in the control group(P<0.05).The maximum foot load(33.32±17.36 vs.65.47±25.52)and the difference of the weight-bearing ratio between the two groups before and after treatment(0.46±0.22 vs.1.71±0.34)were observed.The group was significantly higher than that of the control group(P<0.05).Conclusion The clinical application of active exercise therapy in the treatment of KOA can significantly improve symptoms and promote patients'balance stability,which is an effective method for the treatment of KOA.
