摘要
目的探索某院医疗服务绩效的DRGs管理效果。方法选取2016年2月至2017年3月4218例某院患者为研究对象,2016年2月至2016年8月2062例为对照组,2016年9月至2017年3月2156例患者为观察组,对照组采用常规服务绩效管理,观察组采用DRGs绩效管理。计算时间消耗指数、费用消耗指数、病例组合指数、学科发展均衡性指数等,医疗服务能力、医疗安全、服务效率等作为绩效评价指标,同时对比低风险疾病死亡率差异。结果观察组病例组合指数与对照组病例组合指数差异无统计学意义(p>0.05),观察组时间消耗指数、费用消耗指数、学科发展均衡指数低于对照组,差异有统计学意义(p<0.05)。观察组低风险疾病死亡率低于对照组,医疗绩效评价总分高于对照组,差异均有统计学意义(p<0.05)。结论 DRGs管理能够有效促进医疗服务绩效的改进,提高医院临床医疗服务能力和服务效率。
Objective To explore the effect of DRGs management for medical service performance in a hospital.Method A total of 4218 patients visiting the hospital from February 2016 to March 2017 were selected as subjects.The 2062 patients from February 2016 to August 2016 were in the control group,receiving routine service performance management.The 2156 patients from September 2016 to March 2017 were in the observation group,receiving DRGs performance management.Time consumption index,cost consumption index,case combination index,disciplinary development balance index,etc.were calculated.Medical service ability,medical safety,service efficiency,etc.were chosen as performance evaluation indicators.The difference of mortality of low-risk diseases was compared.Results There was no significant difference in the composite index between the observation group and the control group(p>0.05).The time consumption index,cost consumption index and discipline development balance index of the observation group were lower than the control group,and the difference was statistically significant(p<0.05).The mortality of low-risk diseases in observation group was lower than that of the control group,and the total score of medical performance evaluation was higher than that of the control group(p<0.05).Conclusion DRGs management can effectively improve the performance of medical services,clinical medical service capacity,and service efficiency of hospitals.
