摘要
目的探析髋关节置换术治疗老年股骨颈骨折和股骨头坏死的效果。方法 76例老年股骨颈骨折和股骨头坏死患者作为研究对象,随机分为对照组和观察组,每组38例。对照组实施人工股骨头置换术治疗,观察组实施髋关节置换术治疗。观察比较两组患者的临床疗效及治疗前后髋关节功能评分。结果经治疗后,观察组的治疗优良率为97.37%,明显高于对照组的84.21%,差异有统计学意义(χ2=3.934, P=0.047<0.05)。两组患者治疗前的髋关节功能评分比较差异无统计学意义(P>0.05)。观察组和对照组治疗后的髋关节功能评分分别为(91.16±2.35)分、(82.64±2.47)分,均明显高于治疗前的(42.58±2.36)分、(42.99±2.48)分,差异均有统计学意义(P<0.05),且观察组患者的髋关节功能评分明显高于对照组,差异有统计学意义(P<0.05)。结论在老年股骨颈骨折和股骨头坏死的临床治疗中,采用髋关节置换术进行治疗可获得理想疗效,能显著改善患者髋关节功能,值得临床进一步推广应用。
Objective To discuss and analyze the effect of hip replacement on the treatment of femoral neck fracture and femoral head necrosis in the elderly.Methods A total of 76 elderly patients with femoral neck fracture and femoral head necrosis as study subjects were randomly divided into control group and observation group,with 38 cases in each group.The control group was treated with artificial femoral head replacement,and the observation group was treated with hip replacement.The clinical efficacy and hip function score before and after treatment between the two groups were observed and compared.Results After treatment,the observation group had obviously higher treatment excellent rate as 97.37%than 84.21%in the control group,and the difference was statistically significant(χ2=3.934,P=0.047<0.05).Both groups had no statistically significant difference in hip function score before treatment(P>0.05).The observation group and control group after treatment had obviously higher hip function score respectively as(91.16±2.35)and(82.64±2.47)points than(42.58±2.36)and(42.99±2.48)points before treatment,and the differences were statistically significant(P<0.05).The observation group had obviously higher hip function score than the control group,and the difference was statistically significant(P<0.05).Conclusion Application of hip replacement show ideal efficacy in clinical treatment of femoral neck fracture and femoral head necrosis in the elderly,and it can significantly improve the hip function of patients.So it is worthy of clinical promotion and application.
