摘要
目的:探讨醋酸泼尼松片(强的松)联合布洛芬缓释胶囊治疗亚急性甲状腺炎的安全性与有效性。方法:选取2015年1月至2017年6月在内蒙古鄂尔多斯市中心医院接受治疗的96例亚急性甲状腺炎患者,采用随机数字表法将患者分为对照组和观察组,每组48例。对照组给予口服强的松治疗,观察组给予口服强的松联合布洛芬缓释胶囊治疗。对比两组疗效、血清学指标、甲状腺疼痛消退时间、发热消退时间、肿胀消退时间、不良反应及复发情况。结果:观察组有效率为95.83%,明显高于对照组83.33%(P<0.05);两组肿胀消退时间比较差异无统计学意义(P>0.05),观察组甲状腺肿胀消退时间及发热消退时间均明显短于对照组(均P<0.05);治疗后两组C反应蛋白(CRP)、血沉(ESR)、游离三碘甲腺原氨酸(FT3)及游离甲状腺素(FT4)均呈明显下降趋势(均P<0.05),但观察组下降幅度更为明显(P<0.05)。治疗后两组促甲状腺激素(TSH)呈显著上升趋势(P<0.05),且观察组上升幅度更大(P<0.05)。对照组不良反应发生率为20.83%,明显高于观察组(6.25%)(P<0.05);对照组复发率为6.25%,与观察组(0.00%)比较差异无统计学意义(P>0.05)。结论:强的松联合布洛芬缓释胶囊治疗亚急性甲状腺炎可显著提高疗效,缩短患者甲状腺肿胀消退时间与发热消退时间,降低炎症反应、改善甲状腺功能、降低不良反应发生率,且不增加复发率。
Objective:To investigate the safety and efficacy of prednisone combined with ibuprofen sustained-release capsules in the treatment of subacute thyroiditis.Methods:A total of96patients with subacute thyroiditis treated in our hospital from January2015to June2017were selected,and were randomly assigned to receive either prednisone acetate tablets(control group,n=48)or prednisone combined with ibuprofen sustained-release capsules(observation group,n=48).The efficacy,time to the disappearance of thyroid pain,time to the subsidence of fever,time to swelling mitigation,the incidence of adverse reactions and recurrence rate were compared.Results:The effective rate of the observation group was95.83%,which was significantly higher than that of the control group(83.33%)(P<0.05).There was no significant difference in time to swelling mitigation between the two groups(P>0.05).The time to the disappearance of thyroid pain and time to the subsidence of fever in the observation group were significantly shorter than those in the control group(P<0.05).After treatment,the levels of CRP,ESR,FT3and FT4in both groups were decreased,while the TSH level was increased,and the changes in the observation group were more obvious(P<0.05).The incidence of adverse reactions in the control group was20.83%,which was significantly higher than that in the observation group(6.25%)(P<0.05).The recurrence rate was6.25%in the control group,which was similar to that of observation group(0.00%)(P>0.05).Conclusion:Prednisone combined with ibuprofen sustained-release capsules was more effective for the treatment of subacute thyroiditis.This combination could shorten the thyroid swelling and remission time and fever subsided time,inhibit inflammation reaction,improve thyroid function,reduce the incidence of adverse reactions,and did not increase the recurrence rate.
