摘要 目的探讨泛素特异化家族酶(USP)4和USP14在烧伤创面修复过程中的表达及意义。方法选择2015年1月至2018年3月在我院就诊的大面积烧伤患者作为研究对象,伤后1、3、7、14d时局部麻醉下采用眼科角膜环钻切取组织标本,采用免疫组织化学染色法检测USP4、USP14、转化生长因子(TGF)-β1的表达,观察USP4、USP14、TGF-β1与温哥华瘢痕量表(VSS)评分的相关性。结果创面组织1、3、7、14d时USP4、USP14、TGF-β1表达均高于正常组织,创面组织3d时USP4、USP14、TGF-β1表达较1d时升高,7d时USP4、USP14、TGF-β1表达较3d时升高,14d时USP4、USP14、TGF-β1表达较7d时降低,差异均有统计学意义(P <0.05)。以14d时TGF-β1与7d比较是否降低作为分组依据,将患者分为升高组和降低组,升高组患者VSS水平显著高于降低组,差异均有统计学意义(P <0.05)。VSS评分与14d时USP4、USP14和TGF-β1水平均呈正相关(r=0.617、0.605、0.672,P均<0.05)。结论 USP4和USP14与烧伤创面的修复密切相关,创伤修复后期USP4和USP14异常表达可能和瘢痕形成密切相关,有望成为烧伤后瘢痕防治的新思路。 Objective To investigate the expression and significance of USP4and USP14in the process of burn wound repair.Methods The large area burn patients in our hospital from January2015to March2018were selected as study subjects.The corneal tissue samples were harvested under local anesthesia on the1st,3rd,7th and14th day after injury.The expressions of USP4,USP14and TGF-β1were detected by immunohistochemical staining,and the correlation between USP4,USP14,TGF-β1and Vancouver Scar Scale(VSS)score was observed.Results The expression of USP4,USP14and TGF-β1in wound tissue was higher than that in normal tissue on day1,3,7and14,the expression of USP4,USP14and TGF-β1in wound tissue on day3was higher than that in day1,the expression of USP4,USP14and TGF-β1on day7was higher than that on day3,and the expression of USP4,USP14and TGF-β1on day14was lower than that on day7,the differences were statistically significant(P<0.05).The patients were divided into two groups according to whether TGF-β1was lower on the14th day compared with the7th day,and the VSS level in the elevated group was significantly higher than that in the lower group(P<0.05).The VSS score was positively correlated with the levels of USP4,USP14and TGF-β1at14days(r=0.617,0.605,0.672,P<0.05).Conclusion The USP4and USP14are closely related to the repair of burn wounds,and the abnormal expression of USP4and USP14may be closely related to the formation of scars in the late stage of wound repair,which may become a new idea for the prevention and treatment of burn scars.
