摘要
目的探讨带旋骼深血管蒂髂骨移植治疗股骨头坏死的效果。方法数字随机法选取2014年6月—2017年9月期间该院接收并诊治的股骨头坏死患者50例,根据不同治疗原则将其分为对照组:采用传统髓芯减压加骨移植治疗、研究组:采用带旋骼深血管蒂髂骨移植治疗,每组均25例,对比两组各项手术指标、Harris评分及优良率等。结果两组平均手术时间对比差异无统计学意义(t=0.244 9,P=0.531 5>0.05);但研究组平均出血量、卧床时间、拆线时间均低于对照组(t=-24.570 5、-7.096 6、-18.040 7,P=0.038 8、0.044 6、0.035 6<0.05);研究组治疗1、3个月后的Harris评分高于对照组对比结果显示(t=14.089 4、-19.083 7,P=0.041 8、0.038 6<0.05);研究组优良率高于对照组(χ~2=4.500 0,P=0.033 8<0.05)。结论对股骨头坏死患者应用带旋骼深血管蒂髂骨移植治疗,效果理想,不仅缩短治疗时间,还利于患处快速愈合,避免股骨头继续塌陷。
Objective To study the effect of iliac bone graft with deep pedicled circumflex artery in treatment of femoral head necrosis.Methods50cases of patients with femoral head necrosis admitted and treated in our hospital from October2014to September2017were selected and randomly divided into two groups according to different treatment principles,the control group and the research group were respectively treated with traditional core decompression and bone transplantation and iliac bone graft with deep pedicled circumflex artery,and various operative indexes,Harris score and excellent and good rate were compared between the two groups.Results The difference in the average operation time between the two groups was not statistically significant(t=0.2449,P=0.5315>0.05);and the average bleeding volume,time in bed,time of removing sutures in the research group were lower than those in the control group(t=-24.5705,-7.0966,-18.0407,P=0.0388,0.0446,0.0356<0.05),and the Harris scores in1month and3months after treatment in the research group were higher than those in the control group(t=14.0894,-19.0837,P=0.0418,0.0386<0.05);and the excellent and good rate in the research group was higher than that in the control group(χ^2=4.5000,P=0.0338<0.05).Conclusion The effect of iliac bone graft with deep pedicled circumflex artery in treatment of femoral head necrosis is ideal,which can not only shorten the treatment effect but also benefit the rapid healing and avoid the continuous collapse of the femoral head.
