摘要
目的利用单光子发射型计算机体层摄影(SPECT/CT)骨显像技术研究颞下颌关节盘前移位(ADD)患者髁突骨代谢变化,探讨盘移位时间及程度对髁突骨代谢影响规律。方法选取临床及磁共振成像(MRI)检查确诊为单侧颞下颌关节盘前移位患者130例,纳入病例均接受颞下颌关节SPECT/CT检查,对健患侧感兴趣区(ROI)核素计数值与病程及关节盘移位分期进行配对检验及相关性分析。结果 SPECT/CT检查患侧髁突延迟相异常浓聚阳性率为86.92%(113/130),研究对象健侧(22,13)与患侧ROI计数(26,15)的差异有统计学意义(Z=8.909,P<0.001),患侧ROI计数与盘移位时间呈负相关(r=-0.332,P<0.001)。结论通过SPECT/CT研究得出,随颞下颌关节盘移位时间的推移,ADD髁突成骨性活动降低。
Objective To evaluate the bone metabolism of condyles in patients with anterior disc displacement(ADD)of temporomandibular joint(TMJ)by using SPECT/CT(single photon emission computed tomography/computed tomography)and investigate the objective law between the bone metabolism changes,medical history and states.Methods Inclusion objects consisted of130patients definitely diagnosed as ADD by clinical features and MRI.All patients underwent SPECT/CT on the TMJ before treatment.At last,paired tests and correlated analysis were performed.Results It was showed that the positive rate of condylar delayed phase accumulation was86.92%(113/130).In130cases,the difference of counts of region of interest(ROI)between the healthy(22,13)and affected sides(26,15)was statistically significant(Z=8.909,P<0.001);there was a negative correlation between ROI counts and course of ADD(P<0.001,correlation coefficient r=-0.332).Conclusion It is suggested that the osteogenic activity of the condyle with ADD decreased with the medical history in the SPECT/CT.
