摘要
目的探讨冠心病危重症患者应用主动脉球囊反搏术的护理。方法选择我院2017年10月～2018年6月应用主动脉球囊反搏术治疗的冠心病危重症患者90例,随机分为对照组和实验组,每组45例,对照组予护理常规,实验组予系统化全面护理,比较两组患者满意度、主动脉球囊反搏术实施时间、生命体征稳定的时间,然后分析护理前后两组的左心室射血分数、APACHEII评分、NYHA心功能分级以及出血、球囊导管移位等的发生率。结果实验组满意度高于对照组,P<0.05;实验组主动脉球囊反搏术实施时间、生命体征稳定的时间优于对照组,P<0.05;护理前两组左心室射血分数、APACHEII评分、NYHA心功能分级相似,P>0.05;护理后实验组左心室射血分数、APACHEII评分、NYHA心功能分级优于对照组,P<0.05。实验组出血、球囊导管移位等的发生率低于对照组,P<0.05。结论应用主动脉球囊反搏术治疗的冠心病危重症者实施系统化全面护理效果好,可改善病情和提高心功能,缩短手术时间,稳定生命体征,提高满意度,减少出血、球囊导管移位等的发生率。
Objective To explore the nursing of critical coronary heart disease patients with aortic balloon counterpulsation.MethodsNinety patients with critical coronary heart disease who were treated with aortic balloon counterpulsation in our hospital fromOctober2017to June2018were selected as the study subjects.They were randomly divided into control group and experimentalgroup,with45cases in each group.The control group was given routine nursing care,and the experimental group was givensystematic and comprehensive nursing care.The satisfaction of patients in the two groups,the implementation time of aortic ballooncounterpulsation and the stable time of vital signs were compared.Then,the incidence of left ventricular ejection fraction,APACHEIIscore,NYHA cardiac function grade,bleeding and balloon catheter displacement in the two groups before and after nursing wereanalyzed.Results The satisfaction of the experimental group was higher than that of the control group(P<0.05).The duration ofaortic balloon counterpulsation and the stability of vital signs in the experimental group were better than those in the control group(P<0.05).Left ventricular ejection fraction,APACHEII score and NYHA cardiac function grade were similar in the two groups before nursing,P>0.05.After nursing,left ventricular ejection fraction,APACHEII score and NYHA cardiac function grade of theexperimental group were better than those of the control group,P<0.05.The incidence of bleeding and balloon catheter displacementin the experimental group was lower than that in the control group(P<0.05).Conclusion The systemic and comprehensive nursingeffect of severe coronary heart disease patients treated with aortic balloon counterpulsation is good,which can improve the conditionand cardiac function,shorten the operation time,stabilize vital signs,improve satisfaction,reduce the incidence of bleeding andballoon catheter displacement.
